Glans issues?
Dear Doctor: In recent days, after urination, the skin at the urethral opening and the inner side of the glans tends to stick to my underwear due to a small amount of sticky discharge.
When I pull them apart, there is a slight pain at the glans opening, and touching the glans also causes discomfort.
There is no redness or swelling, and I have not engaged in sexual activity recently.
There is no issue with phimosis.
What could be the possible cause, Doctor?

Hello: 1) If the discharge is coming from the urethra, a urinalysis should be performed, and it is advisable to drink plenty of water first.
If the discharge is produced from the skin, check for any wounds or abrasions.
If there are any, seek medical attention; if not, maintain cleanliness and dryness.
After urination, if convenient, gently pat dry.
If performing any self-care, do not be too vigorous, and avoid doing it too frequently.
Wishing you good health.

More Info
Dear Patient,
Thank you for reaching out with your concerns regarding sensitivity and discomfort in the glans and urethral area. It’s important to address these symptoms as they can be indicative of various underlying conditions. Based on your description, there are several potential causes for your discomfort, and I will outline them along with possible solutions.
Possible Causes
1. Irritation or Inflammation: The symptoms you describe, such as the glans being sensitive and painful upon touch, could be due to irritation or inflammation. This can occur from various sources, including soaps, detergents, or personal hygiene products that may not be suitable for sensitive skin. Even the fabric of your underwear can contribute to irritation if it is too rough or if you are allergic to the material.
2. Balanitis: This is an inflammation of the glans penis, which can be caused by infections (bacterial or fungal), poor hygiene, or irritants. Symptoms often include redness, swelling, and discomfort. While you mentioned there is no visible redness or swelling, it is still possible to have a mild case of balanitis that may not present with these symptoms.
3. Urethritis: This is an inflammation of the urethra, which can be caused by infections (including sexually transmitted infections) or irritants. Even if you have not engaged in sexual activity recently, some infections can remain dormant or be transmitted through non-sexual means.
4. Phimosis or Preputial Stenosis: Although you mentioned that you do not have a problem with phimosis, it’s worth noting that any tightness or difficulty in retracting the foreskin can lead to discomfort and sensitivity.
5. Allergic Reactions: Sometimes, allergic reactions to lubricants, condoms, or even certain foods can manifest as sensitivity in the genital area.
6. Skin Conditions: Conditions such as eczema or psoriasis can also affect the genital area, leading to discomfort and sensitivity.
Solutions and Recommendations
1. Hygiene Practices: Ensure that you are practicing good hygiene. Gently clean the area with warm water and a mild, unscented soap. Avoid using harsh soaps or products that may irritate the skin.
2. Avoid Irritants: Pay attention to any products you are using, including laundry detergents and personal care items. Switching to hypoallergenic products may help alleviate irritation.
3. Moisturizing: If dryness is a concern, consider using a gentle, fragrance-free moisturizer to keep the skin hydrated.
4. Consult a Specialist: Given your symptoms, it would be wise to consult a urologist or a dermatologist who specializes in genital health. They can perform a thorough examination and may recommend tests to rule out infections or other underlying conditions.
5. Avoid Sexual Activity: Until you have a clearer understanding of the cause of your discomfort, it may be best to avoid any sexual activity to prevent further irritation or potential transmission of infections.
6. Medication: If an infection is diagnosed, your doctor may prescribe antibiotics or antifungal medications, depending on the cause.
7. Follow-Up: If symptoms persist or worsen, follow up with your healthcare provider for further evaluation and management.
In conclusion, while your symptoms may be concerning, many potential causes can be effectively managed with appropriate care. It’s essential to seek professional medical advice to ensure a proper diagnosis and treatment plan. Please take care, and I hope you find relief soon.
