Can I get pregnant? What is the probability of getting pregnant?

Hello doctor, my last two menstrual cycles were from April 6 to April 12 and from April 28 to May 5.
I had unprotected intercourse with my boyfriend on May 7, and he withdrew before ejaculation.
What is the likelihood of pregnancy? (My usual menstrual cycle is 28 days; the last one was 36 days, and this one was 22 days.)

Celina, 20~29 year old female. Ask Date: 2021/05/07

Dr. Huang Yuande reply Obstetrics and Gynecology

If the menstrual cycle is irregular, it is not possible to use the calendar method for contraception.
Although this may seem unlikely, it is still advisable to use other methods of contraception.

More Info

When considering the chances of pregnancy after unprotected intercourse, several factors come into play, including the timing of the menstrual cycle, ovulation, and the lifespan of sperm. Based on the information you've provided, let's break down the situation.

Your menstrual cycles have varied in length, with the last two being 28 days and 36 days, respectively. Typically, ovulation occurs about 14 days before the start of your next period. For a standard 28-day cycle, ovulation usually happens around day 14. However, since your cycles have been irregular, it can be challenging to pinpoint the exact day of ovulation.

On May 7, you had unprotected intercourse. If we consider your last menstrual period (LMP) from April 28 to May 5, your next expected period would be around May 26 (assuming a 28-day cycle). If we take the average of your previous cycles, ovulation could have occurred anywhere between May 10 and May 14, depending on the length of your cycle. Sperm can live inside the female reproductive tract for up to five days, which means that if you ovulated shortly after your intercourse, there is a possibility of conception.

Given that you had unprotected sex on May 7, and if ovulation occurred around May 10 to May 14, the chances of pregnancy are relatively high. However, if your ovulation occurred later than that, the chances would decrease.
It's also important to consider that irregular cycles can make it difficult to predict ovulation accurately. If your cycle continues to be irregular, it may be beneficial to track ovulation signs, such as changes in cervical mucus or using ovulation predictor kits, to better understand your fertile window.

If you are concerned about the possibility of pregnancy, you may want to wait until your next expected period and take a home pregnancy test for confirmation. Most home pregnancy tests are reliable when taken after a missed period, as they detect the hormone hCG, which is produced after a fertilized egg implants in the uterus.

In summary, based on your cycle history and the timing of intercourse, there is a reasonable chance of pregnancy. If you are actively trying to conceive or wish to avoid pregnancy, consider discussing contraceptive options with your healthcare provider or exploring methods to track your ovulation more accurately. If you have further questions or concerns about your menstrual cycle or reproductive health, don't hesitate to reach out to a healthcare professional for personalized advice.
