White spots in the pubic hair area?

Hello, doctor.
I previously noticed many small white spots (slightly raised) in the pubic hair area.
They were not painful or itchy, so I didn't pay much attention to them.
However, I recently discovered that they have started to slowly spread to the penis (still not painful or itchy).
What could this condition be?

Wuming, 10~19 year old female. Ask Date: 2021/05/09

Dr. Wu Yuxin reply Dermatology

Without actually seeing the lesion, it is impossible to determine.
If it is very concerning, please seek medical attention as soon as possible.

When it comes to the appearance of white bumps in the pubic area, there are several potential causes that one should consider. The fact that these bumps are not painful or itchy is a positive sign, but it is still essential to understand what they could be and whether any further action is necessary.

One common cause of white bumps in the pubic area is folliculitis, which is an inflammation of the hair follicles. This condition can occur due to irritation, infection, or blockage of the follicles. It often presents as small, raised bumps that may be red or white and can sometimes be mistaken for acne. Folliculitis is generally harmless and can resolve on its own, but maintaining good hygiene and avoiding tight clothing can help prevent further irritation.

Another possibility is pearls of Fordyce, which are small, painless, pale bumps that can appear on the genital area. These are sebaceous glands that are visible through the skin and are completely normal. They do not require treatment unless they cause concern for cosmetic reasons.

Pearly penile papules are another benign condition that can occur on the penis. These are small, white or skin-colored bumps that form around the corona of the glans and are considered a normal anatomical variation. They are not infectious and do not require treatment.

In some cases, sexually transmitted infections (STIs) such as herpes or human papillomavirus (HPV) can cause white bumps or lesions in the genital area. If the bumps change in appearance, become painful, or are accompanied by other symptoms such as discharge or fever, it is crucial to seek medical attention. Early diagnosis and treatment are essential for managing STIs effectively.

Lichen sclerosus is another condition that can cause white patches or bumps in the genital area, particularly in women. This chronic inflammatory skin condition can lead to thinning of the skin and may cause discomfort. If you notice any changes in the skin texture or color, or if you experience any pain or itching, it is advisable to consult a healthcare provider.

Given that the white bumps you are experiencing are spreading and have moved to the penis, it is essential to have them evaluated by a healthcare professional. While many causes of white bumps are benign, a proper diagnosis is necessary to rule out any serious conditions. A dermatologist or urologist can perform a physical examination and may recommend further tests if needed.

In summary, while the presence of white bumps in the pubic area can often be attributed to benign conditions such as folliculitis, Fordyce spots, or pearly penile papules, it is essential to monitor any changes in the bumps and seek medical advice if there are any concerns. Early intervention can help address any potential issues and provide peace of mind. Always prioritize your health and do not hesitate to reach out to a medical professional for guidance.
