I have been experiencing nosebleeds for two and a half years, almost three years?

Frequent nosebleeds occur primarily on the right side, often waking me up in the middle of the night or happening right after I get out of bed.
Occasionally, I experience nosebleeds while showering, and sometimes when I blow my nose, I notice blood streaks or clots.
This has been happening for almost two and a half to three years.
Initially, I had nosebleeds 2-3 times a week, and I have been seeing a traditional Chinese medicine practitioner who suggested that my nosebleeds are due to allergies causing the mucous membranes to become too thin.
Over time, the frequency decreased to 3-10 times a month.
I have a history of nasal allergies and rosacea.
Since October 30, 2020, I have been taking isotretinoin (to treat rosacea).
In the 2-3 months before starting isotretinoin, I experienced nosebleeds 2-3 times a week, but after taking traditional Chinese medicine, the frequency adjusted back to once or twice a week.
Recently, due to the pandemic and my profession, I have been doing weekly rapid tests, and for the past three weeks, I have had nosebleeds daily, sometimes more than once a day, with a maximum of three times.
Previously, I could sense when a nosebleed was about to occur, but in the last 3-4 days, I have had no warning and they happen unexpectedly.

Hello: Common causes of nasal mucosa abrasion and bleeding are related.
If bleeding persists, you can first insert a cotton ball and pinch the nostrils to apply pressure for hemostasis.
Once the pandemic is over, it is recommended to visit an otolaryngologist to assess whether electrocautery treatment is necessary.
Wishing you safety and health.

More Info

Understanding persistent nosebleeds can be quite complex, as they can arise from various underlying causes and may require a multifaceted approach to treatment. Based on your description, it seems that you have been experiencing recurrent nosebleeds primarily from the right nostril for nearly three years, with episodes occurring during sleep, upon waking, and even while showering. Additionally, you mentioned the presence of blood-streaked mucus and occasional clots, which can be concerning.

Causes of Persistent Nosebleeds
1. Nasal Anatomy and Environment: The nasal cavity is lined with a delicate mucous membrane that can become easily irritated. Factors such as dry air, especially during winter months or in air-conditioned environments, can lead to dryness and cracking of the nasal membranes, making them more susceptible to bleeding.
2. Allergies and Sinus Issues: You mentioned having nasal allergies and rosacea, which can contribute to inflammation and irritation of the nasal passages. Allergic rhinitis can cause the nasal membranes to swell and become more fragile, leading to frequent nosebleeds.

3. Medications: The use of topical medications, such as A-acid (isotretinoin), can also affect the nasal mucosa. While isotretinoin is primarily used for acne and rosacea, it can lead to dryness and irritation of mucous membranes, potentially exacerbating nosebleeds.

4. Underlying Health Conditions: Conditions such as hypertension (high blood pressure) or clotting disorders can also lead to increased frequency and severity of nosebleeds. If you have a history of such conditions, it would be prudent to discuss this with your healthcare provider.

5. Structural Issues: Deviated septum or other structural abnormalities in the nasal cavity can predispose individuals to localized bleeding.

Treatment Options
1. Moisturizing the Nasal Passages: Using saline nasal sprays or gels can help keep the nasal membranes moist, reducing the likelihood of bleeding. This is particularly important in dry environments.

2. Avoiding Irritants: If you are aware of specific allergens or irritants that trigger your symptoms, such as smoke or strong odors, avoiding these can help minimize nasal irritation.

3. Medication Review: Since you are on isotretinoin, it may be worth discussing with your doctor whether this medication could be contributing to your nosebleeds. They may suggest adjusting the dosage or switching to a different treatment for rosacea.

4. Allergy Management: If allergies are a significant factor, managing them with antihistamines or other allergy medications may help reduce nasal inflammation and subsequent bleeding.

5. Medical Evaluation: Given the frequency and sudden onset of your nosebleeds, especially if they are occurring without warning, it would be advisable to consult an ear, nose, and throat (ENT) specialist. They can perform a thorough examination, possibly including nasal endoscopy, to identify any structural issues or other underlying causes.

6. Cauterization: In some cases, if a specific blood vessel is identified as the source of the bleeding, cauterization (a procedure to seal the blood vessel) may be recommended.

7. Blood Pressure Monitoring: If you have concerns about hypertension, regular monitoring and management of blood pressure can be crucial in preventing nosebleeds.

Conclusion
Persistent nosebleeds can be distressing and may indicate underlying issues that require attention. It is essential to maintain open communication with your healthcare providers, including both your primary care physician and any specialists, to ensure a comprehensive approach to your symptoms. Keeping a diary of your nosebleed occurrences, noting any potential triggers or associated symptoms, can also be helpful in managing your condition effectively.
