Nosebleed
Hello: My sister has had habitual nosebleeds since she was young.
She has seen a doctor who said it is due to thin nasal membranes.
However, when she has a nosebleed, it lasts for half an hour to an hour, and each time there are different sizes of blood clots.
Sometimes, after one side stops bleeding, the other side starts shortly after.
Strangely, the doctor even mentioned that if she has a nosebleed, her head should be kept low and not elevated, otherwise the blood could flow back into her mouth.
As her older sister, it really pains me to see her bleed so much, so I kindly ask Dr.
Tsai to help me understand what to do and the reasons for her nosebleeds.

Hello, sister: The common cause of nosebleeds in children is allergic rhinitis, which leads to nasal itching and frequent nose picking, resulting in the rupture of small blood vessels.
The treatment method must first address the symptoms of allergic rhinitis.

Frequent nosebleeds, medically known as epistaxis, can be a distressing condition, especially for those who experience them regularly. Understanding the underlying causes and potential solutions is essential for managing this issue effectively.
Causes of Frequent Nosebleeds
1. Thin Nasal Mucosa: As your sister's doctor mentioned, having thin nasal membranes can lead to increased susceptibility to bleeding. The nasal mucosa is delicate and can easily be damaged by various factors, leading to frequent nosebleeds.
2. Environmental Factors: Dry air, especially during winter months or in arid climates, can dry out the nasal passages, making them more prone to bleeding. Additionally, exposure to irritants such as smoke, strong odors, or allergens can exacerbate the condition.
3. Nasal Trauma: Frequent nose picking, blowing the nose forcefully, or any trauma to the nose can cause bleeding. Children, in particular, may not be aware of the potential harm caused by these actions.
4. Underlying Health Conditions: Certain medical conditions, such as blood clotting disorders (like hemophilia or von Willebrand disease), hypertension, or even certain medications (like anticoagulants) can contribute to frequent nosebleeds.
5. Anatomical Issues: Structural problems within the nasal cavity, such as a deviated septum or the presence of nasal polyps, can lead to recurrent bleeding.
Management and Solutions
1. Moisturizing the Nasal Passages: Using saline nasal sprays or humidifiers can help keep the nasal passages moist, reducing the likelihood of bleeding. This is particularly important in dry environments.
2. Avoiding Irritants: Keeping your sister away from smoke, strong odors, and allergens can help minimize irritation to the nasal mucosa.
3. Gentle Nose Care: Encourage her to avoid picking her nose or blowing it forcefully. If she needs to blow her nose, she should do so gently.
4. Positioning During a Nosebleed: The advice given by the doctor about keeping the head low during a nosebleed is generally correct. Leaning forward helps prevent blood from flowing down the throat, which can cause nausea or vomiting. Pinching the nostrils together and applying pressure for about 10 minutes can also help stop the bleeding.
5. Consultation with an ENT Specialist: If the nosebleeds persist or worsen, it may be beneficial for your sister to see an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination to rule out any anatomical issues or underlying health conditions that may be contributing to the problem.
6. Potential Medical Treatments: In some cases, cauterization (a procedure to seal the blood vessels) may be recommended if the bleeding is frequent and severe. This is typically done in a clinical setting by a healthcare professional.
7. Monitoring and Record Keeping: Keeping a record of when the nosebleeds occur, their duration, and any associated activities or environmental factors can provide valuable information for healthcare providers.
Conclusion
While frequent nosebleeds can be alarming, understanding the causes and implementing preventive measures can significantly reduce their occurrence. It’s essential to approach this issue with care and to seek professional medical advice when necessary. Encourage your sister to maintain a healthy environment for her nasal passages and to be mindful of any habits that may contribute to her condition. If the problem persists, further evaluation by a specialist will be crucial in finding a long-term solution.
