Post-operative issues after circumcision?
Hello, doctor.
I had traditional surgery three months ago, but there is still swelling around the area.
During my follow-up appointment last week, my surgeon said the wound has healed and that it is not edema but rather fat that hasn't dissipated (I am overweight).
He advised against another surgery as it could result in a shortened foreskin.
I feel troubled about this; what should I do?

Post-circumcision concerns, especially regarding swelling and recovery, can be quite common and may lead to anxiety for many patients. It’s important to understand the healing process and the factors that can affect recovery after circumcision.
Firstly, it’s reassuring to hear that your surgeon has confirmed that the wound has healed properly. This is a crucial step in the recovery process. However, persistent swelling or a feeling of fullness around the surgical site can be concerning. Your surgeon's assessment that the swelling is not due to edema but rather due to fat that has not dissipated is an important distinction. This can happen, especially in individuals who are overweight, as excess fat can sometimes create a bulging appearance around the surgical area.
Here are some considerations and recommendations for managing your concerns:
1. Weight Management: Since your surgeon mentioned that your weight could be contributing to the appearance of swelling, focusing on a healthy diet and regular exercise may help reduce overall body fat. This could potentially improve the appearance of the area around the circumcision site. Consult with a healthcare provider or a nutritionist for personalized advice on weight management.
2. Follow-Up with Your Surgeon: It’s essential to maintain open communication with your surgeon. If you have ongoing concerns about the appearance of the area or if you experience any discomfort, do not hesitate to schedule a follow-up appointment. Your surgeon can provide further evaluations and may suggest additional treatments or interventions if necessary.
3. Avoiding Further Surgery: Your surgeon has advised against further surgery due to the risk of shortening the remaining skin. This is a valid concern, as additional surgeries can lead to complications and may not yield the desired aesthetic results. It’s crucial to weigh the risks and benefits of any surgical intervention carefully.
4. Managing Discomfort: If you experience any discomfort or pain, over-the-counter pain relief medications, such as ibuprofen or acetaminophen, can be used as directed. However, always consult with your healthcare provider before taking any medication, especially if you have underlying health conditions.
5. Monitoring for Complications: Keep an eye out for any signs of complications, such as increased redness, warmth, or discharge from the surgical site, which could indicate an infection. If you notice any of these symptoms, contact your healthcare provider immediately.
6. Patience in Recovery: Healing after circumcision can take time, and it’s not uncommon for some swelling to persist for several months. The body needs time to adjust after surgery, and factors such as individual healing rates and body composition can influence recovery.
7. Support and Counseling: If the appearance of the area is causing significant distress, consider speaking with a mental health professional. They can provide support and coping strategies to help you manage any anxiety or concerns related to your body image.
In conclusion, while it can be frustrating to deal with persistent swelling after circumcision, it’s essential to focus on overall health and recovery. Maintain regular communication with your healthcare provider, and consider lifestyle changes that can support your healing process. Remember that recovery is a journey, and it’s important to be patient with your body as it heals.
