Itching of the foreskin?

Hello, Doctor! (I am currently abroad) I have noticed that after washing my penis once every one or two weeks, the foreskin starts to itch.
I began washing my penis daily with soap about two to three weeks ago, and it has improved significantly.
However, if I skip a day of washing, the itching returns, and each time I wash, there is a white substance inside the foreskin (this used to happen only after a long time without washing, and it wouldn't itch).
I also noticed that the foreskin has become very prone to tearing if I rub it hard (previously, it wouldn't tear even with vigorous rubbing).
My last sexual encounter was about a month and a half ago, and my partner has no abnormalities in their genital area.
I would like to ask what might be wrong with my penis :) and how to treat it? (I currently have clotrimazole and miconazole ointments on hand).
Thank you, Doctor!

Hello: It is recommended to keep the area clean and dry, and to avoid using soap for now.
Rinse with water in the morning and evening, gently pat dry, and make sure to dry the area after urination.
Avoid tight underwear and change it daily.
Following this advice, you should see significant improvement within a week.

More Info

It sounds like you're experiencing some discomfort related to your foreskin, which can be caused by a variety of factors. Let's break down the symptoms and potential treatments for penile foreskin itching and irritation.

Understanding the Symptoms
1. Itching and Irritation: Itching of the foreskin can be due to several reasons, including poor hygiene, infections (like yeast infections or sexually transmitted infections), or skin conditions such as eczema or contact dermatitis.

2. White Discharge: The white substance you’re noticing could be smegma, which is a combination of dead skin cells, oils, and moisture that can accumulate under the foreskin if not cleaned regularly. While smegma itself is not harmful, its presence can indicate inadequate cleaning, which may lead to irritation or infection.

3. Easily Broken Skin: If the skin on your foreskin is becoming fragile and easily broken, this could be due to irritation from excessive washing or the use of harsh soaps. The skin may also be inflamed or infected, which can lead to increased sensitivity.

Recommended Treatment
1. Hygiene Practices: It’s crucial to maintain proper hygiene. However, washing the penis too frequently or using harsh soaps can strip the skin of its natural oils, leading to dryness and irritation. Instead, aim to wash gently with warm water and a mild, unscented soap. It’s generally recommended to clean the area daily, but avoid over-washing.

2. Topical Treatments: Since you have clotrimazole (often used for fungal infections) and miconazole (another antifungal), these can be helpful if a yeast infection is suspected. Apply these creams as directed, typically twice a day for a week. If you notice improvement, continue using them until the symptoms resolve.

3. Avoid Irritants: Avoid using scented soaps, lotions, or other products that may irritate the sensitive skin of the genital area. If you suspect that a product is causing irritation, discontinue its use.

4. Moisturizing: If the skin is dry, consider using a gentle, fragrance-free moisturizer to help soothe the area. This can help prevent cracking and irritation.

5. Consult a Healthcare Provider: If symptoms persist despite these measures, or if you notice any unusual discharge, pain, or other concerning symptoms, it’s important to consult a healthcare provider. They can perform a physical examination and may recommend further tests or treatments, such as antifungal medications or topical steroids if inflammation is present.

6. Sexual Health Considerations: Since you mentioned that your last sexual encounter was a month and a half ago and your partner has no symptoms, it’s less likely that this is a sexually transmitted infection. However, if you have concerns about your sexual health or if symptoms worsen, seeking medical advice is essential.

Conclusion
In summary, maintaining proper hygiene without over-washing, using mild products, and applying antifungal creams as needed are key steps in managing your symptoms. If the situation does not improve or worsens, please seek medical attention to rule out any underlying conditions. Your health and comfort are important, and a healthcare professional can provide tailored advice and treatment options.
