Itching and pain of the penis?

Hello Doctor: I previously experienced discomfort, pain, and itching in my penis, so I consulted several doctors who diagnosed me with eczema and allergies.
Blood tests showed no issues.
A few weeks ago, I noticed ulceration on the surface of my glans, so I went to see a doctor again.
The doctor checked my blood sugar and said it was normal, and prescribed me zinc oxide ointment and medication.
Over a month has passed, and the wound has mostly healed, but I still experience itching and pain.
The doctor mentioned that it might be due to the sensitivity of the healing area and that it should improve as it heals.
I was advised to return for a circumcision, but I am still worried because this condition has improved and then recurred several times.
At one point, I was concerned that I might have a serious illness.
For the past couple of days, I have been experiencing pain during urination.
Is this sensitivity, or could it be inflammation or an infection? Additionally, I find it a bit difficult to clean the zinc oxide ointment.
If the effectiveness is similar, can I switch back to Meikang? I apologize for having so many questions, but this has been troubling me for a long time.
Thank you, Doctor, for your patience in answering my concerns; I greatly appreciate it!

R: Hello, could it be that you are over-cleaning, which has caused damage to the epidermis? You can refer to the method I provided for cleaning the glans.
1.
Clean the glans several times a day.
2.
It is best to clean away any smegma after each urination; washing is preferable, but if that's not convenient, simply patting dry is sufficient.
3.
Cleaning method: * use only cold water Absolutely do not use soap, body wash, or any cleaning agents.
If you feel dryness, you can apply a small amount of petroleum jelly, baby oil, or a gentle, non-irritating moisturizer.
If you have any further questions, feel free to ask again, and we can discuss and research together.
Online responses can only serve as a reference; if necessary, you should seek medical attention at a healthcare facility.

Understanding penile itching and pain can be quite complex, as there are numerous potential causes and treatments. Based on your description, it seems you have been experiencing a series of symptoms that include itching, pain, and sensitivity in the penile area, particularly after a period of ulceration on the glans. Here’s a comprehensive overview of possible causes, treatments, and considerations regarding your situation.

Possible Causes
1. Dermatitis or Eczema: As you mentioned, some doctors have diagnosed you with eczema. This condition can cause itching, redness, and inflammation. Allergic reactions to soaps, detergents, or even certain fabrics can exacerbate these symptoms.

2. Infections: Bacterial or fungal infections can lead to similar symptoms. Conditions like candidiasis (yeast infection) or bacterial infections can cause itching, pain, and even discharge. If you have a history of recurrent infections, this may be a contributing factor.

3. Sexually Transmitted Infections (STIs): While you may have been tested, it’s essential to rule out STIs, as they can present with symptoms of pain and itching. Conditions like herpes or gonorrhea can cause discomfort in the genital area.

4. Sensitivity or Irritation: After an injury or ulceration, the skin can become more sensitive. This heightened sensitivity can lead to discomfort during urination or when the area is touched.

5. Phimosis or Balanitis: If you have not undergone circumcision, conditions like phimosis (tight foreskin) or balanitis (inflammation of the glans) can lead to pain and discomfort, especially if there is a history of poor hygiene or irritation.

6. Psychological Factors: Anxiety about your symptoms can sometimes exacerbate the perception of pain and discomfort. Stress can lead to increased sensitivity and awareness of bodily sensations.

Treatment Options
1. Topical Treatments: You mentioned using zinc oxide, which is often used for its protective and soothing properties. If you find it difficult to clean, you might consider alternatives like hydrocortisone cream (for inflammation) or antifungal creams if a fungal infection is suspected. Always consult your doctor before switching treatments.

2. Hygiene Practices: Maintaining proper hygiene is crucial. Gently clean the area with mild soap and water, and ensure it is thoroughly dried afterward. Avoid using harsh soaps or products that can irritate the skin.

3. Avoid Irritants: Identify and avoid any potential irritants, such as certain soaps, lotions, or fabrics that may be causing allergic reactions.

4. Medical Follow-Up: Since you have experienced recurrent symptoms, it may be beneficial to follow up with a urologist or dermatologist who specializes in genital conditions. They can provide a more thorough examination and possibly conduct additional tests to rule out infections or other underlying issues.

5. Consider Circumcision: If phimosis or recurrent balanitis is a significant issue, circumcision may be a recommended option. This procedure can help alleviate symptoms and reduce the risk of future infections.

6. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, can help manage discomfort. However, consult your doctor before taking any new medications.

Conclusion
Your symptoms of penile itching and pain can arise from various causes, including dermatitis, infections, and sensitivity following an injury. It’s essential to maintain good hygiene, avoid irritants, and consult with a specialist for a thorough evaluation. If your symptoms persist or worsen, further investigation may be necessary to ensure proper treatment and management. Remember, it’s crucial to communicate openly with your healthcare provider about your concerns and any changes in your symptoms.
