Epididymitis
Hello, Doctor.
At the end of March, I visited a urology clinic due to intermittent pain in my left testicle.
During the examination, the doctor noted that pressing on a specific point caused significant pain.
He diagnosed me with epididymitis and prescribed a three-day course of antibiotics, after which I returned for a follow-up appointment and was given another three days of antibiotics.
As of now, I still occasionally experience slight pain in my left testicle, but it usually subsides after a while.
I also feel that my left testicle is smaller, and when I apply the same pressure to both testicles, the left side is more painful.
Additionally, when I sleep on my right side, I often have the sensation that my left testicle is dropping.
I have several questions, Doctor:
1.
Are my symptoms due to incomplete resolution of the epididymitis?
2.
I often pull back the foreskin to cover the glans during masturbation, and I recently learned that this might cause retrograde ejaculation.
Can retrograde ejaculation also lead to epididymitis?
3.
Given the current pandemic, I am hesitant to seek medical care.
Are there any lifestyle changes I can make to improve my condition?
4.
I have not engaged in sexual intercourse yet.
Does this significantly increase my chances of infertility?
(Sorry for the many questions, and thank you for your help.)

Hello:
1.
Is my symptom due to incomplete recovery from epididymitis? --> It is possible, but the likelihood is low.
Theoretically, if inflammation persists, it would become increasingly swollen.
2.
I often pinch the foreskin upwards to cover the glans during masturbation, and I recently learned that this might cause semen reflux...
Does semen reflux also easily lead to epididymitis? --> It shouldn't, but it's not advisable to do this.
3.
During the pandemic, I'm hesitant to seek medical care.
Are there any methods to improve my condition at home? --> Gentle exercise, avoiding spicy foods, and reducing DIY activities.
4.
I have not engaged in sexual intercourse yet.
Does this significantly increase my chances of infertility? (Sorry for the many questions, thank you, doctor.) --> It will not.
Wishing you good health.

More Info
Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. It can be caused by various factors, including bacterial infections, sexually transmitted infections (STIs), or even non-infectious causes such as trauma or certain medications. Understanding the symptoms, treatment options, and concerns related to epididymitis is crucial for effective management and recovery.
Symptoms of Epididymitis
The symptoms of epididymitis can vary, but common signs include:
1. Pain and Swelling: Typically, the pain is localized to the affected side (in your case, the left side) and may be accompanied by swelling of the epididymis and testicle.
2. Tenderness: As you experienced, pressing on the epididymis can elicit significant pain.
3. Discomfort During Sexual Activity: Pain may also occur during ejaculation or sexual activity.
4. Changes in Testicular Size: Some individuals may notice that the affected testicle appears smaller or feels different compared to the other side.
5. Other Symptoms: These may include fever, chills, or a discharge from the penis if an infection is present.
Treatment Options
The treatment for epididymitis often involves:
1. Antibiotics: As you were prescribed, a course of antibiotics is typically the first line of treatment, especially if a bacterial infection is suspected.
2. Pain Relief: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage discomfort.
3. Rest and Support: Elevating the scrotum and wearing supportive underwear can alleviate discomfort.
4. Follow-Up Care: It’s essential to follow up with your healthcare provider to ensure the infection has resolved and to address any ongoing symptoms.
Addressing Your Concerns
1. Incomplete Recovery: The lingering pain and discomfort you are experiencing could indicate that the epididymitis has not fully resolved. It’s important to communicate these symptoms to your healthcare provider, as they may need to reassess your condition and possibly extend your antibiotic treatment or investigate other underlying issues.
2. Semen Reflux and Epididymitis: Regarding your concern about semen reflux caused by the way you handle your penis during masturbation, while this is generally not a direct cause of epididymitis, improper techniques or excessive pressure could potentially irritate the epididymis. If you notice pain or discomfort, consider adjusting your technique.
3. Self-Care During the Pandemic: If you are hesitant to seek in-person medical care due to the pandemic, consider the following self-care measures:
- Rest: Avoid strenuous activities that could exacerbate your symptoms.
- Ice Packs: Applying ice packs to the affected area can help reduce swelling and pain.
- Hydration: Staying well-hydrated can support overall health and recovery.
- Monitor Symptoms: Keep track of any changes in your symptoms, and seek medical attention if they worsen or do not improve.
4. Fertility Concerns: As for your concern about infertility, while epididymitis can affect sperm quality and transport, the impact on fertility depends on the severity and duration of the condition. Since you have not engaged in sexual intercourse, your risk of infertility is likely low at this stage. However, if you continue to experience symptoms or if the condition persists, it would be wise to discuss your concerns with a healthcare provider who can offer more personalized advice and potential fertility assessments.
Conclusion
Epididymitis can be a distressing condition, but with appropriate treatment and care, most individuals recover fully. It’s essential to maintain open communication with your healthcare provider about your symptoms and any concerns you may have. If your symptoms persist or worsen, do not hesitate to seek medical attention, even during the pandemic, as telehealth options may also be available to provide you with the care you need.
