Ankle pain. Numbness extending to the hand as well?

Hello, Doctor.
I apologize for bothering you.
My child has been complaining of leg pain since he was four years old.
At first, we thought it was growing pains, but after a while, it didn't seem right.
When I asked him, he said his legs feel numb, but it's only around the ankles.
(I previously found that many cases of growing pains occur in the joints or tend to happen in the afternoon or evening before bedtime.) After realizing something was wrong, we took him to rehabilitation, orthopedics, pediatrics, and pediatric therapy.
He has had X-rays and blood tests, but everything came back normal.
He is now six and a half years old, and recently the symptoms have worsened.
He often becomes so numb that he falls to the ground, rolling and kicking around, unable to stand or sit.
Recently, he has also started experiencing numbness and weakness in his hands.
This happens multiple times, both during the day and at night, whether he is happily playing, going out to buy things, or just sitting and waiting to play with his toys.
He suddenly becomes weak and falls to the ground, and I have to support him with both hands.
I have researched but am unsure what to do next.
Could you please help me with this? Or let me know which specialist I should consult? Thank you for reading.

Understanding ankle pain and numbness in children can be a challenging experience for parents, especially when the symptoms persist and escalate over time. Based on your description, it sounds like your child has been experiencing significant discomfort and episodes of numbness in the ankles and hands, which has raised concerns for both you and the healthcare professionals involved in their care.

Possible Causes of Ankle Pain and Numbness
1. Growing Pains: While you initially suspected growing pains, it's important to note that these typically occur in the late afternoon or evening and are often self-limiting. They usually do not cause numbness or significant functional impairment.

2. Nerve Compression or Injury: Numbness and weakness can indicate nerve compression or injury. Conditions such as tarsal tunnel syndrome, where the tibial nerve is compressed at the ankle, could lead to symptoms like numbness and pain.
3. Circulatory Issues: Poor circulation can also lead to numbness and weakness. This could be due to various reasons, including vascular anomalies or other underlying conditions that affect blood flow.

4. Neurological Conditions: Conditions affecting the nervous system, such as peripheral neuropathy or other neurological disorders, could also manifest as numbness and weakness in the limbs.

5. Musculoskeletal Issues: Structural problems in the ankle or foot, such as flat feet or other deformities, could lead to pain and discomfort, potentially causing compensatory mechanisms that result in numbness.

6. Systemic Conditions: Certain systemic conditions, such as diabetes or autoimmune disorders, can lead to neuropathy and should be ruled out, especially if there are other symptoms present.

Recommended Steps for Parents
1. Consult a Pediatric Neurologist: Given the complexity and persistence of your child's symptoms, it would be prudent to consult a pediatric neurologist. They specialize in conditions affecting the nervous system and can conduct more comprehensive evaluations, including nerve conduction studies or electromyography (EMG) to assess nerve function.

2. Physical Therapy: Engaging a physical therapist who specializes in pediatric care may help in managing symptoms and improving strength and coordination. They can provide exercises tailored to your child's needs and help with functional mobility.

3. Regular Monitoring: Keep a detailed log of your child's symptoms, including the frequency, duration, and triggers of the episodes. This information can be invaluable for healthcare providers in diagnosing the issue.

4. Consider a Multidisciplinary Approach: Sometimes, a team approach involving pediatricians, neurologists, orthopedists, and physical therapists can provide a more comprehensive understanding of the issue and lead to better management strategies.

5. Emotional Support: Chronic pain and discomfort can be distressing for children. Providing emotional support and reassurance can help them cope with their symptoms. Engaging in calming activities and ensuring they feel safe during episodes can also be beneficial.

Conclusion
Your child's symptoms of ankle pain and numbness warrant thorough investigation, especially given their persistence and the recent escalation in severity. While initial assessments have not revealed any abnormalities, it is crucial to continue seeking answers through specialized care. A pediatric neurologist will be best equipped to explore potential neurological causes, while a multidisciplinary approach may provide comprehensive support for your child's physical and emotional well-being. Remember, early intervention can lead to better outcomes, so do not hesitate to pursue further evaluations.
