Irregular menstrual periods after a medical abortion?
Dear Doctor,
I had a medical abortion in mid-February.
My first menstrual period after the abortion was on April 2nd, lasting five days.
I then had my periods on May 1st, May 15th, and May 28th, each lasting three days.
Recently, on June 20th, I had a one-day period and then it stopped, with June 21st being very light.
I have a thyroid condition and am currently on medication to manage it.
Given this situation, should I continue to monitor my menstrual flow, or should I see a doctor? Thank you for your assistance.

After undergoing a medical abortion, it is not uncommon for women to experience irregular menstrual cycles as their bodies adjust to the hormonal changes. In your case, it appears that you have had a series of menstrual cycles that have varied in length and flow, which can be expected following such a procedure.
Typically, after a medical abortion, it may take some time for your menstrual cycle to return to its regular pattern. The first few cycles can be particularly irregular, and this can be influenced by various factors, including hormonal fluctuations, stress, and any underlying health conditions, such as your thyroid issue.
From your description, it seems that your menstrual cycles have been somewhat erratic, with periods occurring at different intervals and varying in duration. This is not unusual, especially in the first few months following an abortion. The fact that you had a five-day period followed by several shorter cycles indicates that your body is still adjusting.
However, there are a few important considerations to keep in mind:
1. Thyroid Health: Since you mentioned having a thyroid condition, it is crucial to ensure that your thyroid levels are well-managed. Thyroid dysfunction can significantly impact menstrual cycles, leading to irregularities. If you are on medication for your thyroid, make sure you are adhering to your treatment plan and having regular check-ups to monitor your thyroid function.
2. Monitoring Your Cycle: It is advisable to continue observing your menstrual cycle for a few more months. Keep track of the length, flow, and any other symptoms you may experience. This information can be helpful if you need to consult with a healthcare provider later.
3. When to Seek Medical Advice: If you notice any of the following, it would be wise to consult your healthcare provider:
- If your periods do not normalize within three to six months.
- If you experience heavy bleeding or severe pain during your cycles.
- If you have any unusual symptoms, such as persistent abdominal pain, fever, or unusual discharge, which could indicate an infection or other complications.
4. Emotional and Physical Well-being: The emotional and physical aspects of recovering from an abortion can also affect your menstrual cycle. Ensure you are taking care of your mental health and seeking support if needed.
In summary, while it is generally acceptable to continue monitoring your menstrual cycle for now, do not hesitate to reach out to your healthcare provider if you have concerns or if your cycles do not stabilize in the coming months. Regular follow-ups, especially considering your thyroid condition, will help ensure that any potential issues are addressed promptly.
