Nipple issues in the breast?


The nipples on both sides appear to be covered with a layer that resembles a 'film' (not completely) that can be peeled off but is somewhat painful, as if it is connected to the skin.
There are many lesions that look like chronic eczema (nipple lichen planus).
They are usually somewhat hard, but exposure to hot water and applying lotion can soften them slightly.
Since there is no pain or itching, I haven't addressed it (it's been a few years).
I'm unsure if this is related to sweating easily and not specifically cleaning the chest area, but it is aesthetically unpleasing, and I am hesitant to ask others or family members.
Due to the recent pandemic, I haven't been able to see a doctor.
