Nipple Issues: Wrinkles, Itching, and Skin Peeling - Dermatology

Chest issues

I have been experiencing wrinkling of my nipples for some unknown reason (they tend to become more wrinkled when I rub, scratch, or come into contact with saliva), along with itchiness and persistent peeling (the dark brown skin of the nipple peels every two to three days).
Occasionally, there is a secretion of a liquid that I suspect might be breast milk, although I am certain I am not pregnant.
This liquid has a natural, pleasant scent that is not from my clothing or any chemical factors, and it can appear suddenly at times.
Sometimes, when I scratch, my nipples feel a bit moist, as if liquid is being secreted.
These symptoms have persisted for several years, but I did not have this issue before high school.
I would like to know what factors might be causing this.
I have asked my family members (mother, aunt, grandmother), and they have never encountered this problem, so the likelihood of it being hereditary seems low.

What troubles me the most is the wrinkling and itching.
I previously consulted a dermatologist about this, and he suggested that it was simply due to dehydration, recommending that I drink more water and maintain good moisturization by applying lotion.
However, I have been consistently applying lotion after my daily showers and reapplying it when my nipples are wrinkled and dry.
In terms of hydration, many of my friends drink as much water as I do, yet they do not experience similar issues, which makes me question whether this is purely a dehydration problem.
My condition has not improved.
I apologize for the lengthy description and appreciate your assistance.
Thank you.

It is not possible to determine without actually seeing the lesion, and it is essential to confirm whether it is simply a secretion or truly breast milk, as the diseases considered are entirely different.
Please seek medical attention as soon as possible.

More Info

Understanding nipple issues such as wrinkles, itching, and skin peeling can be quite complex, as these symptoms can arise from various underlying factors. Based on your description, it seems that you are experiencing a combination of symptoms that have persisted for several years. Let's break down the potential causes and management strategies for your concerns.

Possible Causes
1. Dry Skin: The skin on the nipples can become dry and flaky due to environmental factors, such as cold weather, low humidity, or frequent exposure to irritants (like soaps or detergents). Dry skin can lead to itching and peeling, which may explain the symptoms you are experiencing.

2. Eczema or Dermatitis: Conditions like eczema or contact dermatitis can affect the nipple area, leading to symptoms such as itching, redness, and peeling. These conditions can be triggered by allergens or irritants, including certain fabrics, soaps, or even hormonal changes.

3. Fungal Infections: Yeast infections can occur in warm, moist areas of the body, including the nipple region. Symptoms may include itching, redness, and sometimes a discharge that could be mistaken for milk.

4. Hormonal Changes: Hormonal fluctuations, especially during puberty, menstruation, or other life stages, can affect the skin's moisture levels and sensitivity. This might explain why you did not experience these issues before high school.

5. Nipple Discharge: The clear, pleasant-smelling fluid you describe could be a normal secretion from the Montgomery glands, which are small glands located around the nipple. However, if the discharge is accompanied by other symptoms, it might warrant further investigation.

Management Strategies
1. Moisturization: Continue to apply a gentle, fragrance-free moisturizer to the nipple area after bathing. Look for products that contain ceramides or hyaluronic acid, which can help restore the skin barrier and retain moisture.

2. Avoid Irritants: Be mindful of the products you use on your skin. Opt for mild, hypoallergenic soaps and avoid fabric softeners or detergents that may cause irritation. Wearing breathable, cotton clothing can also help reduce friction and irritation.

3. Hydration: While you mentioned that you drink plenty of water, ensure that you are also consuming a balanced diet rich in essential fatty acids (found in fish, nuts, and seeds) to support skin health.

4. Consult a Dermatologist: Since you have already seen a dermatologist, it may be beneficial to revisit them or seek a second opinion if your symptoms persist. They may recommend a topical steroid for inflammation or a specific treatment for eczema or dermatitis.

5. Monitor Symptoms: Keep track of any changes in your symptoms, including the frequency and nature of the discharge. If you notice any unusual changes, such as a change in color, odor, or consistency of the discharge, or if you develop new symptoms, it is important to seek medical attention promptly.

Conclusion
While your symptoms may initially seem like a simple case of dry skin, they could be indicative of a more complex issue that requires professional evaluation. It is essential to approach this matter with a comprehensive understanding of potential causes and management strategies. If your symptoms continue to cause discomfort or concern, do not hesitate to seek further medical advice. Your health and comfort are paramount, and a healthcare professional can provide tailored recommendations based on a thorough examination.
