Urethral itching
Hello, I underwent a series of sexually transmitted infection tests last year, including blood and urine tests, all of which were normal.
I have a stable sexual partner and we have been together for a long time.
I also had an MRI of the pelvis and prostate, which were normal.
I do not smoke, drink alcohol, or consume coffee.
Question 1: Recently, I have been experiencing itching in the urethra without any discharge.
The itching occurs intermittently during the day; sometimes it is in the anterior urethra, sometimes in the mid to posterior urethra.
Occasionally, I need to apply pressure to the urethra or perineal area to feel the itch, but at times the symptoms disappear.
It usually does not itch during urination, but the itching is more pronounced at night or upon waking.
I took a week of antifungal medication without improvement, and after taking a week of probiotics, there was some slight improvement, but the daytime itching still appears and disappears, while the nighttime and morning itching persists.
Question 2: Typically, after having sexual intercourse for two to three consecutive days, I feel discomfort in the perineal area or prostate, but it improves after a few days of rest.
I have also experienced ejaculation with blood in the semen.

Based on your description, it still sounds like a urinary tract infection.
It could be that the initial urinary tract infection was not completely treated, or that there was a lack of simultaneous treatment between you and your partner, leading to cross-infection.
Both possibilities exist.
I recommend that you see a urologist for an examination; otherwise, trying random medications may lead to antibiotic resistance.
Hematospermia is generally benign, but if you have concerns, it is still advisable to consult a urologist and discuss it with your physician.

More Info
Urethral itching can be a perplexing and uncomfortable condition, and understanding its potential causes is crucial for effective management and relief. Based on your description, it seems you have undergone thorough medical evaluations, including tests for sexually transmitted infections (STIs) and imaging studies, which have returned normal results. This is a positive sign, but it does not eliminate the possibility of other underlying issues that could be contributing to your symptoms.
Possible Causes of Urethral Itching
1. Irritation or Allergic Reactions: One common cause of urethral itching is irritation from soaps, lotions, or laundry detergents. If you have recently changed any personal care products, consider reverting to your previous products to see if symptoms improve. Additionally, some individuals may have sensitivities to certain fabrics or materials, particularly in underwear.
2. Infections: While your STI tests were negative, other infections such as yeast infections or bacterial infections can cause similar symptoms. Even in the absence of discharge, these infections can lead to localized itching and discomfort.
3. Prostatitis: Given your history of discomfort in the perineal area and prostate, prostatitis (inflammation of the prostate gland) could be a contributing factor. This condition can cause a variety of symptoms, including discomfort in the pelvic area, urethral itching, and sometimes even urinary symptoms.
4. Nerve Sensitivity: Sometimes, nerve irritation or sensitivity in the pelvic region can lead to sensations of itching or discomfort. This could be related to pelvic floor dysfunction or other musculoskeletal issues.
5. Diet and Lifestyle Factors: Although you mentioned not consuming alcohol, caffeine, or tobacco, dietary factors can still play a role. Spicy foods, acidic foods, and certain artificial sweeteners can sometimes exacerbate urinary tract symptoms.
Management and Solutions for Relief
1. Avoid Irritants: As mentioned, identify and avoid any potential irritants in your personal care products. Opt for hypoallergenic products and wear breathable, cotton underwear.
2. Hydration: Ensure you are well-hydrated, as this can help dilute urine and reduce irritation during urination.
3. Warm Baths: Soaking in a warm bath can provide temporary relief from itching and discomfort. Adding Epsom salts may also help soothe irritation.
4. Topical Treatments: Over-the-counter hydrocortisone cream may provide relief from itching, but it should be used sparingly and not for prolonged periods without consulting a healthcare provider.
5. Consult a Urologist: Given the persistence of your symptoms, it may be beneficial to consult a urologist who can perform a more detailed evaluation. They may consider additional tests, such as a cystoscopy, to visualize the urethra and bladder for any abnormalities.
6. Pelvic Floor Therapy: If nerve sensitivity or pelvic floor dysfunction is suspected, working with a physical therapist specializing in pelvic health may provide significant relief.
7. Monitor Sexual Activity: Since you noted discomfort after consecutive days of sexual activity, consider taking breaks between sexual encounters to see if this alleviates your symptoms.
8. Follow-Up on Blood in Semen: The experience of blood in your semen (hematospermia) should be discussed with a healthcare provider, as it can sometimes indicate underlying issues that may need further investigation.
Conclusion
Urethral itching can stem from various causes, and while your initial tests have ruled out some serious conditions, it is essential to continue exploring other potential factors. By addressing irritants, maintaining good hydration, and consulting with specialists, you can work towards finding relief from your symptoms. If the itching persists or worsens, do not hesitate to seek further medical advice. Your health and comfort are paramount, and a tailored approach will be key to managing this condition effectively.
