Postoperative wound
I am the mother of a child who had an appendectomy on June 5th.
Recently, I noticed that the right side of the incision looks a bit like a keloid, but it feels soft when pressed.
My child says it hurts (normally, he doesn't complain of pain).
Given the current pandemic situation, I am inquiring whether this condition requires medical attention.
Thank you.

Dr. Ke Fangxu reply Surgery
If it has been a month since the surgery, there should generally be no issues.
Surgical site infections typically occur within the first week post-operation, while intra-abdominal abscesses may develop around two weeks later.
If there are no symptoms of fever or abdominal pain, there is likely no need to rush to seek medical attention.

Post-surgery wound care is crucial for ensuring proper healing and preventing complications. In your case, your child underwent an appendectomy on June 5, and you are observing some concerning signs at the surgical site. The appearance of the wound resembling a keloid (or crab foot-like) and your child's report of pain are significant factors to consider.
Firstly, it’s important to understand that some degree of pain and swelling is normal after surgery, especially in the first few days as the body begins to heal. However, if the pain persists or worsens, or if the wound shows signs of unusual changes, it may indicate a complication. Keloids are raised scars that can develop after surgery, but they typically do not cause pain unless they are irritated or inflamed. The fact that your child is experiencing pain at the site of the wound is a concern that should not be overlooked.
Here are some key points to consider regarding when to seek medical attention:
1. Increased Pain: If your child is experiencing significant pain that is not alleviated by over-the-counter pain medications, or if the pain is worsening rather than improving, it is advisable to consult a healthcare professional.
2. Signs of Infection: Look for signs of infection, which can include increased redness, warmth, swelling, or discharge from the wound. If the wound appears to be oozing pus or has a foul odor, these are clear indicators that medical attention is needed.
3. Changes in Wound Appearance: If the wound is becoming more raised, hard, or discolored, this could indicate a keloid formation or other complications. While keloids themselves are not harmful, they can be uncomfortable and may require treatment if they cause pain or restrict movement.
4. Fever or Systemic Symptoms: If your child develops a fever or exhibits other systemic symptoms such as chills, fatigue, or loss of appetite, these could be signs of a more serious infection that requires immediate medical evaluation.
5. Follow-Up Appointments: Ensure that you are attending all scheduled follow-up appointments with your child's surgeon. These visits are essential for monitoring the healing process and addressing any concerns that may arise.
Given the current pandemic situation, it is understandable to be cautious about seeking medical care. However, it is crucial to prioritize your child's health and well-being. If you have any doubts about the wound's condition or your child's pain, it is better to err on the side of caution and consult a healthcare provider. Many clinics and hospitals have implemented safety measures to minimize the risk of COVID-19 transmission, and telemedicine options may also be available for initial consultations.
In summary, while some discomfort and changes in the surgical site can be normal, persistent pain, signs of infection, or significant changes in the wound's appearance warrant a medical evaluation. It is essential to keep an open line of communication with your child's healthcare team to ensure a smooth recovery process.
