Post-syphilis infection issues?
Hello doctor, I had a blood test for syphilis at the infectious disease department in mid-May.
A week later, during my follow-up appointment, the doctor confirmed that I was infected with syphilis and herpes simplex virus type 1 and 2.
The syphilis titer was 1:64x+.
The doctor advised me to receive one injection of penicillin and to return for a follow-up in three months.
At that time, I had a rash on my body.
Now, in early July, I frequently experience dizziness, my weight has not increased, and I have developed new rashes or red spots resembling herpes.
I also tested negative for HIV.
I would like to know if my current symptoms of dizziness, weight loss, rashes, and irregular bowel movements are related to untreated syphilis.

Dr. Sun Hongshun reply Urology
Hello Joe! It is recommended that you undergo another HIV blood test (due to concerns about the window period).
Additionally, please test for syphilis again (including both VDRL and TPHA tests), and follow up regularly with the infectious disease department.

It sounds like you have several concerns regarding your health following a diagnosis of syphilis and herpes simplex virus (HSV) infections. Let's break down your symptoms and their potential connections to your previous infections.
First, it's important to understand that syphilis is a sexually transmitted infection caused by the bacterium Treponema pallidum. If left untreated, it can lead to serious complications, including damage to the heart, brain, and other organs. However, since you received treatment with penicillin, the likelihood of these severe complications is significantly reduced, especially if you follow up with your healthcare provider as recommended.
Your symptoms of dizziness, weight loss, and the appearance of rashes or spots on your skin can be concerning. While these symptoms may not directly indicate that your syphilis treatment was ineffective, they could be related to several factors:
1. Post-Treatment Effects: After treatment for syphilis, some individuals may experience a Jarisch-Herxheimer reaction, which is a temporary worsening of symptoms due to the rapid die-off of bacteria. This reaction can include fever, chills, headache, and muscle pain, and it typically occurs within the first 24 hours after treatment. However, this reaction usually resolves quickly.
2. Other Infections: The presence of herpes simplex virus can also cause skin lesions, and if you are experiencing new rashes or spots, it is essential to differentiate whether these are related to HSV or another condition. Herpes outbreaks can lead to discomfort and may be accompanied by systemic symptoms like fatigue or malaise.
3. Nutritional Factors: Weight loss can be attributed to various factors, including stress, anxiety, or changes in appetite due to your recent diagnosis and treatment. It's crucial to maintain a balanced diet to support your immune system and overall health.
4. Mental Health: The psychological impact of an STI diagnosis can lead to anxiety and depression, which can manifest physically as dizziness and changes in appetite or weight. If you are feeling overwhelmed, it may be beneficial to speak with a mental health professional.
5. Gastrointestinal Issues: Irregular bowel movements can stem from stress, dietary changes, or infections. If you are experiencing persistent gastrointestinal symptoms, it may warrant further investigation.
Given your symptoms, it is crucial to follow up with your healthcare provider. They can perform a thorough evaluation, including blood tests and possibly imaging studies, to rule out any complications or co-infections. It is also essential to discuss your ongoing symptoms, as they may require additional treatment or management strategies.
In summary, while your symptoms may not directly indicate that your syphilis treatment was ineffective, they warrant further investigation to ensure your overall health and well-being. Regular follow-ups with your healthcare provider are essential for monitoring your recovery and addressing any new or ongoing concerns.
