White spots on the glans?
Hello doctor, one day I noticed white bumps at the tip of my penis, but they only become noticeable when magnified or pressed hard.
This condition has persisted for a long time without any changes.
I suspect it might be sebaceous gland hyperplasia or pearly penile papules.
It is not painful or itchy, but I am very concerned that it could be genital warts.
Could you please tell me what this looks like? Thank you.

When it comes to the appearance of white bumps on the glans, it's understandable to feel concerned, especially given the sensitive nature of the area. Based on your description, it seems that the bumps are not causing any pain or discomfort, which is a positive sign. However, it’s essential to consider various potential causes for these white bumps.
One common condition that could explain the presence of white bumps on the glans is Pearly Penile Papules (PPP). These are small, harmless, dome-shaped bumps that typically appear around the corona of the glans. They are a normal anatomical variant and are not associated with any disease or infection. PPP is more common in uncircumcised men and does not require treatment unless the individual desires removal for cosmetic reasons.
Another possibility is sebaceous glands or sebaceous cysts, which can appear as small white bumps. These glands are responsible for producing sebum, an oily substance that helps lubricate the skin. If these glands become enlarged or blocked, they can form small cysts that may be visible on the skin.
You mentioned the possibility of Fordyce spots, which are small, painless, pale bumps that can appear on the glans and are actually enlarged sebaceous glands. Like PPP, Fordyce spots are completely benign and do not require any treatment.
On the other hand, your concern about genital warts, which are caused by the human papillomavirus (HPV), is valid. Genital warts can appear as small, flesh-colored or white bumps and may be raised or flat. They can occur in clusters and may sometimes be itchy or uncomfortable. However, the fact that your bumps are not changing in size or causing any discomfort is reassuring, as genital warts often have a more aggressive appearance and can change over time.
It's also worth considering lichen sclerosus, a chronic skin condition that can cause white patches or bumps on the genital area. This condition is more common in men and can lead to scarring if left untreated. However, it typically presents with other symptoms such as itching or discomfort.
Given that you have observed these bumps for a prolonged period without any changes, it is likely that they are benign. However, it is always best to consult a healthcare professional for an accurate diagnosis. A dermatologist or urologist can perform a physical examination and may recommend a biopsy or other tests if there is any doubt about the nature of the bumps.
In summary, while the white bumps on the glans could be due to several benign conditions such as Pearly Penile Papules, Fordyce spots, or sebaceous cysts, it is crucial to rule out any potential sexually transmitted infections or other skin conditions. If you remain concerned, I encourage you to seek a professional evaluation to provide peace of mind and appropriate guidance.
