Pulling back the foreskin during an erection can lead to ejaculation?
Hello, doctor.
I do not have a problem with phimosis; my foreskin does not cover the glans when flaccid or erect, and I can retract it freely.
However, when my penis is erect and engorged, it becomes very hard, and I experience a noticeable pulling sensation at the coronal ridge due to the retraction of the foreskin.
This does not cause discomfort but makes me particularly sensitive.
If I continue to pull the foreskin back with my hand, I will ejaculate; even a few strokes during masturbation will lead to immediate ejaculation.
More concerning is that during intercourse with my partner, I ejaculate as soon as my penis enters the vagina, and this is uncontrollable due to the foreskin being pulled back.
I would like to understand why pulling back the foreskin triggers ejaculation.
Is it related to the length of the foreskin? When my partner uses her mouth, I can last longer, but when using her hand, it feels the same as when I do it myself, and I find it very easy to ejaculate, making it difficult to control.
Thank you, doctor.

Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation during sexual activity, leading to distress for both the individual and their partner. Your inquiry about the role of foreskin sensitivity in this condition is quite pertinent, as sensitivity can indeed play a significant role in sexual arousal and ejaculation.
Firstly, it’s important to understand that the sensitivity of the glans (the head of the penis) and the surrounding areas, including the foreskin, can vary significantly among individuals. In your case, you mentioned that when your penis is erect, the tension and sensitivity at the coronal ridge (the area where the glans meets the shaft) increase, which seems to trigger an urge to ejaculate. This heightened sensitivity can be attributed to several factors, including the presence of nerve endings in the area and the physical sensations associated with the stretching of the foreskin.
The relationship between foreskin sensitivity and premature ejaculation is not entirely straightforward. While some men may experience increased sensitivity due to the presence of foreskin, others may find that circumcision or the absence of foreskin leads to a decrease in sensitivity and potentially better control over ejaculation. However, it’s essential to note that sensitivity is subjective and can be influenced by psychological factors, such as anxiety or excitement, which can also contribute to premature ejaculation.
In your case, the fact that you experience ejaculation with minimal stimulation, whether through manual stimulation or during intercourse, suggests that your sensitivity may be heightened. This could be due to the physical sensations associated with the foreskin being pulled back, which may create a strong reflexive response leading to ejaculation. It’s also worth considering that the psychological aspect of anticipating ejaculation can create a feedback loop, where the anxiety about ejaculating quickly can actually lead to quicker ejaculation.
To address your concerns, here are a few strategies that may help you manage premature ejaculation:
1. Desensitizing Techniques: You might consider using desensitizing creams or sprays that contain numbing agents to reduce sensitivity temporarily. These can be applied to the glans or the shaft before sexual activity.
2. Pelvic Floor Exercises: Strengthening your pelvic floor muscles through Kegel exercises can help improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
3. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you learn to control your arousal levels and delay ejaculation. These methods involve stopping stimulation before ejaculation and allowing the arousal to subside before continuing.
4. Communication with Partner: Open communication with your partner about your experiences and concerns can help reduce anxiety and create a more relaxed sexual environment.
5. Professional Help: If these strategies do not yield satisfactory results, consulting a healthcare professional or a sex therapist can provide tailored advice and treatment options, including cognitive-behavioral therapy or medication.
In conclusion, while foreskin sensitivity may play a role in your experience of premature ejaculation, it is essential to consider the multifaceted nature of this condition, which includes both physiological and psychological components. By exploring various techniques and possibly seeking professional guidance, you can work towards achieving better control over your ejaculation and enhancing your sexual experience.
