Sensitivity Changes in Circumcised Men: A Urology Perspective - Urology

Glans penis

I have undergone a circumcision, so my glans is fully exposed.
However, due to prolonged exposure and constant friction, I no longer feel arousal when touching my glans.
Interestingly, during sexual intercourse, the friction inside the vagina makes me very sensitive, leading to premature ejaculation.

1.
I would like to ask why the sensations during prolonged sexual activity differ from those experienced during self-stimulation of the glans?
2.
I understand that without the protection of the foreskin, the glans is prone to keratinization due to long-term friction.
If keratinization occurs, how can I restore it? Thank you.

Ming, 30~39 year old female. Ask Date: 2009/10/19

Dr. Xu Weikai reply Urology

Hello, DIY and sexual activity are inherently different.
The methods of stimulation and intensity vary, and there are psychological factors involved, making them completely distinct experiences.
Prolonged friction on the glans can indeed lead to keratinization; however, this typically does not affect future sexual activity.
For maintenance, wearing loose-fitting underwear (boxer shorts) can help reduce further friction and improve the situation.
If there are any unclear points, feel free to ask again for further discussion and research.
Sincerely, Dr.

Understanding sensitivity changes in circumcised men is a multifaceted issue that involves anatomical, physiological, and psychological factors. When a man undergoes circumcision, the foreskin, which provides a protective covering for the glans (the head of the penis), is removed. This can lead to several changes in sensitivity and sensation over time.

1. Sensitivity Differences in Sexual Activity vs. Manual Stimulation: After circumcision, many men report changes in sensitivity. The glans, now exposed, may initially be more sensitive due to the lack of the protective foreskin. However, over time, the constant exposure to friction from clothing and other activities can lead to a phenomenon known as keratinization. This process thickens the skin on the glans, potentially reducing sensitivity. As a result, many men find that while they may have diminished sensitivity during manual stimulation, they can experience heightened sensitivity during sexual intercourse. This is often due to the different types of stimulation involved; penetrative sex may engage more nerve endings and create a different sensory experience compared to manual touch.

2. Keratinization and Recovery: Keratinization is a natural response of the skin to constant friction or irritation. In the case of the glans, this can lead to a thickening of the skin, which may dull sensitivity. If you are experiencing this, there are a few approaches you can consider to help restore sensitivity:
- Moisturization: Using a gentle, non-irritating moisturizer can help keep the skin supple and may reduce the thickness of the keratinized layer. Avoid products with fragrances or harsh chemicals.

- Desensitization Techniques: Gradually increasing the amount of stimulation to the glans can help retrain the nerve endings and potentially restore some sensitivity. This can be done through gentle manual stimulation or using sexual aids designed to enhance sensitivity.

- Consultation with a Specialist: If you find that the changes in sensitivity are significantly impacting your sexual health or quality of life, it may be beneficial to consult a urologist or a sexual health specialist. They can provide tailored advice and may suggest treatments or therapies that can help.

3. Psychological Factors: It's also important to consider the psychological aspect of sexual arousal and sensitivity. Anxiety, stress, and performance pressure can all affect sexual response. Engaging in open communication with your partner and exploring different forms of intimacy can help alleviate some of these concerns.

In summary, changes in sensitivity following circumcision are common and can vary based on individual experiences and circumstances. Understanding these changes can help you navigate your sexual health more effectively. If you have ongoing concerns or experience significant discomfort, seeking professional medical advice is always a prudent step.
