Foreskin issues
I would like to ask whether the frenulum should be removed during circumcision.
I am considering the surgery, but I have heard that the frenulum is one of the sensitive areas for men.
If it is removed, will it decrease sensitivity? Will there be a complete loss of sensation? I am also concerned about whether this will affect erectile function or lead to issues later in life.
Additionally, I would like to know what it means if there is a bifurcation in the urine stream.

Hello: 1) It is not always necessary to perform a biopsy.
2) You can discuss with your physician.
Regardless of the area of skin, any wound can potentially alter sensation, but it is unlikely to be completely insensate.
3) The separation of urine is primarily influenced by factors within the urethra.
Wishing you good health.

More Info
Circumcision, or the surgical removal of the foreskin from the penis, is a procedure that has been practiced for various cultural, religious, and medical reasons. When considering circumcision, many individuals have questions regarding the implications of the procedure, particularly concerning the frenulum, sensitivity, and urinary issues.
Frenulum and Sensitivity
The frenulum is a band of tissue located on the underside of the penis that connects the glans (the tip) to the shaft. It is often considered a sensitive area, and many men report that it plays a significant role in sexual pleasure. During circumcision, the frenulum can be preserved or removed, depending on the surgical technique used and the preferences of the patient and surgeon.
1. Sensitivity After Circumcision:
- If the frenulum is removed during circumcision, some men may experience a change in sensitivity. However, it is important to note that sensitivity is subjective and can vary widely among individuals. While some may feel a decrease in sensitivity, others may not notice a significant difference. The glans, which is exposed after circumcision, may become less sensitive over time due to keratinization (the process where the skin becomes thicker and tougher), which can lead to a different sensation during sexual activity.
2. Impact on Sexual Function:
- Concerns about reduced sexual pleasure or erectile function are common. However, studies have shown that most men do not experience a significant decrease in sexual satisfaction after circumcision. In fact, some report improved sexual experiences due to reduced friction and irritation that can occur with an intact foreskin.
Urinary Issues
Regarding urinary problems, such as a split or bifurcated urine stream, this can be indicative of several conditions:
1. Urethral Stricture: This is a narrowing of the urethra that can cause a split stream. It may result from previous infections, trauma, or congenital anomalies.
2. Frenulum Issues: If the frenulum is too tight or causes discomfort, it can lead to abnormal positioning of the urethra, which might affect the urine stream.
3. Other Anatomical Variations: Sometimes, anatomical variations can lead to changes in how urine is expelled, including a bifurcated stream.
Recommendations
- Consultation with a Urologist: Before proceeding with circumcision, it is advisable to have a thorough discussion with a urologist. They can provide insights into the benefits and risks associated with the procedure, including the implications of removing the frenulum.
- Discussing Concerns: Be open about your concerns regarding sensitivity and urinary issues. A urologist can help address these issues and may suggest alternative treatments or surgical techniques that preserve the frenulum if that is a priority for you.
- Post-Operative Care: If you decide to go through with the circumcision, follow-up care is essential. This includes monitoring for any complications and discussing any changes in sensitivity or urinary function with your healthcare provider.
In conclusion, circumcision is a personal decision that should be made after careful consideration of the potential benefits and risks. Understanding the role of the frenulum, the implications for sensitivity, and addressing any urinary issues with a qualified healthcare provider will help you make an informed choice that aligns with your health and lifestyle needs.
