Regarding wisdom tooth extraction?

How long does it take to extract a horizontal wisdom tooth (my wisdom tooth hasn't fully erupted yet)? Also, if a topical anesthetic is applied first, will the injection of local anesthesia still be painful?

Ka Fei, 10~19 year old female. Ask Date: 2021/07/16

Dr. Lin Jiayong reply Dentistry

Hello: Under normal circumstances, the procedure takes about 30 to 50 minutes.
Generally, after surface anesthesia, there should be minimal pain sensation when the needle is inserted (however, some sensation may still be present, including pulling, pushing, and pressure during the tooth extraction process).

When it comes to wisdom tooth extraction, particularly for impacted or horizontally positioned wisdom teeth, the procedure can vary in duration depending on several factors, including the complexity of the extraction, the position of the tooth, and the individual patient's anatomy. Generally, the extraction of a wisdom tooth can take anywhere from 20 minutes to an hour. If the tooth is fully erupted and in a straightforward position, the extraction may be quicker. However, if the tooth is impacted or requires surgical intervention, it may take longer.

Regarding pain management during the procedure, dentists typically use local anesthesia to numb the area around the tooth being extracted. The application of a topical anesthetic before the injection of the local anesthetic can help minimize discomfort from the needle. Most patients report that the injection itself is the most painful part of the procedure, but the use of a topical anesthetic can significantly reduce this sensation.
After the extraction, it is common to experience some pain and swelling. Here are some pain management tips to help you cope with discomfort post-extraction:
1. Follow Post-Operative Instructions: Your dentist will provide specific instructions on how to care for the extraction site. Following these guidelines is crucial for proper healing and pain management.

2. Pain Relief Medications: Over-the-counter pain relievers such as ibuprofen or acetaminophen can be effective in managing post-operative pain. Your dentist may also prescribe stronger pain medications if necessary.

3. Cold Compress: Applying a cold compress to the outside of your cheek near the extraction site can help reduce swelling and numb the area, providing relief from pain.

4. Rest: Ensure you get plenty of rest after the procedure. Avoid strenuous activities for at least 24 hours, as this can exacerbate swelling and discomfort.

5. Dietary Adjustments: Stick to soft foods and avoid hot, spicy, or crunchy foods that may irritate the extraction site. Foods like yogurt, applesauce, and smoothies are good options.

6. Hydration: Stay hydrated, but avoid using straws for the first few days, as the suction can dislodge the blood clot that forms in the extraction site, leading to a painful condition known as dry socket.

7. Oral Hygiene: Maintain good oral hygiene, but be gentle around the extraction site. Your dentist may recommend using a saltwater rinse starting 24 hours after the extraction to help keep the area clean.

8. Monitor for Complications: Keep an eye out for any signs of complications, such as excessive bleeding, severe pain that does not improve with medication, or signs of infection (such as fever or pus). If you experience any of these symptoms, contact your dentist immediately.

In summary, the extraction of wisdom teeth can take anywhere from 20 minutes to an hour, depending on the complexity of the case. Using a topical anesthetic before the injection of local anesthesia can help minimize discomfort. Post-operative pain can be managed effectively with medications, cold compresses, and proper care of the extraction site. Always follow your dentist's instructions for the best recovery experience.
