Cardiac issues
Dear Doctor,
My father is 64 years old and has diabetes, which is usually managed with medication and diet.
Recently, he has been experiencing insomnia at night, accompanied by some chest pain and occasional shortness of breath.
He has seen a cardiologist and had an electrocardiogram, but the doctors said everything is fine.
Sometimes, I really lack confidence in general hospitals and don’t know what to do.
I hope you can help me with this! Thank you!
Sincerely,
A Worried Daughter

Hello Ms.
Chang: Diabetes, hypertension, high cholesterol, smoking, and being male over the age of 45 are risk factors for cardiovascular disease.
The more risk factors present, the higher the likelihood of developing cardiovascular disease.
Coronary artery disease is one of the causes of chest pain; however, a normal electrocardiogram does not rule out the possibility of coronary artery disease.
If coronary artery involvement is suspected, a nuclear scan can be arranged to confirm this (our hospital offers this examination).
In addition, it is essential to manage the risk factors for cardiovascular disease.

Dear Concerned Daughter,
Thank you for reaching out with your concerns about your father's health. It's understandable to feel anxious when a loved one is experiencing health issues, especially when they involve the heart. Your father's situation, given his history of diabetes and recent symptoms, warrants careful consideration and a proactive approach.
Firstly, it's important to recognize that diabetes can significantly impact cardiovascular health. Individuals with diabetes are at a higher risk for heart disease, which can manifest in various ways, including chest pain, shortness of breath, and fatigue. The symptoms you describe—insomnia, chest discomfort, and occasional breathlessness—could potentially be linked to his diabetes, but they may also indicate other underlying conditions that need to be addressed.
Since your father has already seen a cardiologist and had an electrocardiogram (ECG) that returned normal results, it might be beneficial to explore further diagnostic tests. Sometimes, an ECG may not capture transient issues, so additional tests such as an echocardiogram (which uses sound waves to create images of the heart) or a stress test (to evaluate how the heart performs under physical stress) could provide more insight into his heart health.
Moreover, the symptoms of insomnia and anxiety can also contribute to physical sensations that mimic heart issues. Stress and anxiety can lead to increased heart rate, palpitations, and even chest pain. It might be worthwhile to consider whether your father is experiencing heightened stress or anxiety, particularly if there have been recent changes in his life or health status.
In terms of managing his diabetes, ensure that he is adhering to his medication regimen and following a balanced diet. Regular monitoring of blood sugar levels is crucial, as fluctuations can lead to various complications, including cardiovascular issues. Encourage him to engage in regular physical activity, as this can help improve overall health and reduce stress.
If your father continues to experience these symptoms despite normal test results, it may be beneficial to seek a second opinion from another cardiologist or a specialist in internal medicine. Sometimes, a fresh perspective can uncover issues that may have been overlooked.
Additionally, consider discussing his mental health with a healthcare provider. If anxiety or depression is contributing to his symptoms, a mental health professional can provide strategies and support to help him cope.
Lastly, as a daughter, your support is invaluable. Encourage open communication with your father about his feelings and experiences. Sometimes, just having someone to talk to can alleviate some of the stress and anxiety he may be feeling.
In summary, while your father's initial tests may not indicate a serious heart condition, it's essential to remain vigilant and proactive. Further testing, lifestyle modifications, and mental health support can all play a crucial role in managing his health effectively. Don't hesitate to advocate for your father's health and seek the care he needs.
Wishing you and your father all the best in navigating this challenging time.
Sincerely,
