Prostate issues

Hello, doctor.
When my penis is erect, even without any stimulation or with slight local stimulation, I feel a sensitivity inside and have the urge to urinate, which leads to the release of a white prostatic fluid.
After this, I experience a strong sensation of ejaculation, and it happens unexpectedly.
This has been occurring for a while, particularly in the last 1 to 2 years.
Additionally, even in a semi-erect state, I experience the same sensations.
My erections have also become less firm and less rigid.
I would like to ask if this could be due to inflammation of the prostate?

Weiwei, 20~29 year old female. Ask Date: 2021/07/21

Hello: Your condition is not very typical, but if the situation persists, you should seek medical evaluation.
Currently, you can engage in light exercise, drink plenty of water, and avoid spicy and irritating foods.
Alcohol, in particular, should be avoided.
Do not sit for long periods without getting up to move around.
Monitor your condition.
Wishing you good health.

Understanding prostate issues can be quite complex, as they often involve a variety of symptoms and potential underlying causes. Based on your description, it seems you are experiencing a range of symptoms that could be related to prostate health, particularly prostatitis, which is inflammation of the prostate gland.
Prostatitis can manifest in several ways, including pain or discomfort in the pelvic area, difficulty urinating, and changes in sexual function. The symptoms you describe—such as the sensation of needing to urinate during an erection, the release of prostatic fluid, and the feeling of a strong urge to ejaculate—are not uncommon in cases of prostatitis. This condition can lead to increased sensitivity in the pelvic region, which may explain the sensations you are experiencing.

The fact that you have noted these symptoms over the past one to two years suggests a chronic issue, which is often the case with prostatitis. Chronic prostatitis can be particularly challenging to diagnose and treat, as its symptoms can fluctuate and may not always respond to standard treatments. It's also worth noting that prostatitis can be caused by various factors, including bacterial infections, pelvic floor muscle tension, or even psychological factors such as stress and anxiety.

Your mention of experiencing a "half-hard, half-soft" state during erections, along with the associated symptoms, could indicate erectile dysfunction, which is often linked to prostate issues. The prostate plays a significant role in sexual function, and inflammation or other problems in this gland can lead to difficulties in achieving or maintaining an erection. Additionally, the release of prostatic fluid without significant sexual stimulation may suggest heightened sensitivity or dysfunction in the prostate or surrounding tissues.

To address your concerns, it is essential to consult with a urologist who can perform a thorough evaluation, including a physical examination, urine tests, and possibly imaging studies. These assessments can help determine whether you have prostatitis or another underlying condition. Treatment options may include antibiotics if a bacterial infection is suspected, anti-inflammatory medications, pelvic floor physical therapy, or lifestyle modifications such as dietary changes and stress management techniques.

In the meantime, consider the following recommendations:
1. Hydration: Ensure you are drinking enough water, as dehydration can exacerbate urinary symptoms.

2. Diet: Avoid irritants such as caffeine, alcohol, and spicy foods, which can aggravate the bladder and prostate.

3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels may help improve symptoms.

4. Stress Management: Engage in relaxation techniques such as yoga, meditation, or deep-breathing exercises to help alleviate stress, which can contribute to pelvic tension and discomfort.

5. Regular Check-ups: Keep regular appointments with your healthcare provider to monitor your symptoms and adjust treatment as necessary.

In conclusion, while your symptoms may be indicative of prostatitis or another prostate-related issue, a comprehensive evaluation by a healthcare professional is crucial for accurate diagnosis and effective treatment. Don't hesitate to seek medical advice, as early intervention can lead to better outcomes and improved quality of life.
