Visual distortion, perceiving straight lines as curved?
I have approximately 3 to 400 degrees of vision in both eyes and wear glasses.
Recently, I visited two hospitals in the past week where I underwent OCT and fundus examinations.
The results showed that my retina and macula are normal, and I was only advised to pay attention to my eye health.
I was also given two bottles of eye drops, but I do experience distortion in my vision, although it is not blurry.
Could you please tell me what other reasons might cause visual distortion? My family believes it must be due to excessive eye strain, as I have been spending a lot of time looking at screens during my recent vacation.
However, the more I think about it, the more I feel something is not right, and I suspect there might be an issue with my macula or retina.
Doctor, is it really possible that this is just due to eye strain, or could it be related to the brain? Is there a chance that there are undetected changes in the macula or retina?

Dr. He Jianxian reply Ophthalmology
Excessive eye use does not cause visual distortion.
If there are no issues with the macula or retina, vision should not be distorted.
If there is indeed a distortion problem, one must consider causes outside of the eyes.

Visual distortion can be a perplexing symptom, especially when you have already undergone thorough examinations like OCT and retinal imaging that show normal results for your retina and macula. Given your situation, it’s essential to explore various potential causes for the distortion you are experiencing.
Firstly, it is important to understand that visual distortion can arise from several factors, not solely from eye strain or fatigue. While excessive screen time can lead to symptoms like eye strain, dryness, and temporary visual disturbances, it is less common for it to cause persistent visual distortion. The fact that your OCT and retinal examinations returned normal results is reassuring, but it does not completely rule out other underlying issues.
One possible cause of visual distortion is the condition known as visual snow syndrome, which can manifest as a persistent visual disturbance where individuals see "static" or "snow" in their visual field. This condition is often accompanied by other symptoms such as light sensitivity and afterimages. Although the exact cause of visual snow is not well understood, it is thought to be related to neurological processing rather than a direct eye issue.
Another consideration is migraines, particularly those that are accompanied by aura. Some individuals experience visual disturbances, including distortions, as a precursor to a migraine headache. These visual auras can include zigzag patterns, flashing lights, or even temporary loss of vision. If you have a history of migraines, this could be a relevant factor.
Additionally, neurological conditions can also lead to visual distortions. For instance, conditions affecting the brain's visual processing areas, such as lesions or other abnormalities, could potentially cause such symptoms. If you have any accompanying symptoms like headaches, dizziness, or changes in coordination, it would be prudent to consult a neurologist for further evaluation.
Regarding your concern about the macula or retina potentially having issues that were not detected during your examinations, it’s worth noting that while OCT is a highly sensitive tool for detecting many retinal conditions, some subtle changes may not be easily visible. If you continue to experience visual distortions, it may be beneficial to seek a second opinion or request additional imaging studies, such as a fluorescein angiography, which can provide more detailed information about the blood flow in the retina and identify conditions like macular degeneration or diabetic retinopathy that may not be evident on standard OCT.
In summary, while excessive screen time can contribute to visual discomfort, your symptoms of visual distortion warrant a broader investigation beyond just eye strain. It is essential to consider neurological factors, potential migraine-related issues, and the possibility of subtle retinal changes. If your symptoms persist or worsen, I recommend following up with both an ophthalmologist and a neurologist to ensure a comprehensive evaluation of your visual health. Remember, maintaining good eye health practices, such as taking regular breaks from screens and ensuring proper lighting while reading or working, can also help alleviate some discomfort.
