Visual distortion issues

Hello Doctor, at the beginning of March, when I looked at the Amsler grid with my left eye while gazing to the right, I noticed that the squares on the left side appeared distorted.
Later, I found that this distortion also occurred when looking at cabinets.
I underwent a dilated eye exam and discovered a hole, which was immediately treated.
However, even after the treatment, I still experience distortion.
The doctor mentioned that the distortion is unrelated to the hole, but I am certain that the distortion began when I discovered the hole (as I had a dilated exam a month prior with no abnormalities and no distortion).
It has been over four months now; previously, I only felt the distortion when closing my right eye, but in the last two weeks, I have occasionally noticed distortion when using both eyes at certain angles (though it is not as severe as with one eye).
Five days ago, I returned for a dilated exam, and the doctor said my condition has improved.
I also had a brain CT scan three months ago, which showed no abnormalities.
However, I am unsure what could be causing the occasional distortion in my vision with both eyes.
I have a history of health anxiety and tend to magnify my symptoms, which makes me very concerned.

Hello: Possible causes of visual distortion include: 1.
Macular degeneration: The Amsler grid test is primarily used to detect macular degeneration.
After retinal tear repair surgery, some patients may experience changes in the macula, which could be one of the reasons for visual distortion.
Although the doctor states that the distortion is unrelated to the tear, subtle changes in the macula during the recovery period post-surgery may still affect vision.
2.
Vitreous changes: After retinal tear repair, the state of the vitreous may change, and these changes could also lead to visual distortion.
3.
Visual adaptation: After surgery, the eyes need time to adapt to the new visual state.
During this adaptation period, some visual abnormalities may occur, which typically decrease over time.
4.
Psychological factors: Your health anxiety may make you more sensitive to visual abnormalities.
This could heighten your awareness of visual distortion, thereby intensifying your focus on these sensations.
Recommendations for management: 1.
Continuous observation: Regularly visit an ophthalmologist and inform the doctor of any changes in your symptoms.
The doctor may conduct more detailed examinations to determine if there is macular degeneration.
2.
Psychological adjustment: Since you mentioned having health anxiety, it is advisable to discuss this with a psychologist or counselor to seek professional psychological support and treatment, which can help reduce excessive concern and anxiety.
3.
Eye health: Avoid excessive eye strain, maintain good eye habits, take appropriate breaks, and reduce eye fatigue and stress.
4.
Continued monitoring: If the visual distortion worsens or new visual problems arise, seek an ophthalmological examination immediately to ensure the health of the retina and macula.
I hope these suggestions help you understand and manage your current visual issues.
Wishing you a speedy recovery.

More Info

Understanding visual distortion can be a complex issue, especially when it involves changes in vision that may be linked to underlying eye health concerns. Based on your description, it seems you are experiencing visual distortions, particularly in your left eye, which you first noticed after a retinal tear was treated. While your doctor has indicated that the distortion is not related to the tear, it is understandable that you are concerned given the timing of these symptoms.

Visual distortions can arise from various factors, including retinal issues, refractive errors, or even neurological conditions. In your case, since you have already undergone a thorough examination, including dilation and imaging, it is reassuring that no significant abnormalities were found. However, the persistence of the distortion, especially after a recent treatment, warrants further exploration.

One possible explanation for the ongoing distortion could be related to the healing process of the retina. After a retinal tear, the eye undergoes changes as it heals, and sometimes these changes can temporarily affect how visual information is processed. This can lead to phenomena such as metamorphopsia, where straight lines appear wavy or distorted. It is also possible that the brain is still adjusting to the changes in your vision, especially if you have a history of anxiety or health concerns that may amplify your perception of these distortions.

Another factor to consider is the role of binocular vision. You mentioned that the distortion is less severe when using both eyes, which suggests that your brain is attempting to compensate for the discrepancies between the two eyes. This is a normal function of the visual system, but it can sometimes lead to discomfort or confusion if one eye is not providing clear information.

To address your concerns, here are some recommendations:
1. Follow-Up Appointments: Continue to have regular follow-ups with your eye care professional. If the distortion persists or worsens, it may be beneficial to seek a second opinion or additional testing, such as visual field tests or optical coherence tomography (OCT), to assess the retina's condition more closely.

2. Visual Rehabilitation: If the distortion is affecting your daily life, consider consulting with a vision therapist or optometrist specializing in visual rehabilitation. They can provide exercises and strategies to help your brain adapt to the changes in your vision.

3. Manage Anxiety: Since you mentioned having anxiety about your health, it may be helpful to address these feelings through counseling or relaxation techniques. Anxiety can exacerbate the perception of visual disturbances, making them seem more pronounced than they are.

4. Monitor Symptoms: Keep a journal of your symptoms, noting when they occur and any associated activities. This information can be valuable for your eye care provider in understanding the context of your visual distortions.

5. Healthy Eye Practices: Ensure you are practicing good eye health habits, such as taking regular breaks from screens, using proper lighting while reading, and maintaining a balanced diet rich in vitamins beneficial for eye health.

In conclusion, while your visual distortions are understandably concerning, they may be part of the adjustment process following your retinal treatment. Continued monitoring and appropriate interventions can help manage your symptoms and improve your overall visual comfort. Remember, your eye health is paramount, and staying proactive in your care will serve you well.
