Shaking, chills, dizziness, and headaches after orgasm during masturbation?
Dear Doctor,
I am a 36-year-old male.
I usually masturbate about 1-2 times a day, and this frequency has been consistent recently.
Yesterday morning, I also ejaculated without any particular issues.
However, after reaching orgasm last night before bed, I experienced involuntary shaking throughout my body, teeth chattering, chills, and dizziness (similar to the feeling I had when my blood sugar was poorly controlled).
This was accompanied by a headache (in the upper left side of my head), lasting about 5 minutes before gradually subsiding.
I had a similar episode about six months ago, but that one lasted only about 10-15 seconds.
I have a history of hypertension, hyperlipidemia, and diabetes, which are all currently well-controlled.
My dinner the night before was normal, but I did receive the AZ vaccine seven days ago.

Hello, sir: Your symptoms resemble "headache associated with sexual activity," and it is important to first rule out structural lesions such as ruptured cerebral aneurysms causing subarachnoid hemorrhage, arterial dissection, cerebral vasospasm, or brain tumors.
If the examination results are normal, you may need to consider "headache associated with sexual activity" as a type of primary headache.
There are three types of triggers: Type 1 is tension-type headache, which mainly occurs during the pre-orgasm phase; Type 2 is vascular, explosive headache, which primarily occurs at the moment of orgasm; Type 3 is post-coital positional headache, possibly related to low cerebrospinal fluid pressure.
May I ask what your blood pressure is? Do you have any smoking habits? The thrombotic adverse reaction associated with the AZ vaccine is currently referred to as VITT (Vaccine-induced immune thrombotic thrombocytopenia), with an incidence of about 1 in 10,000 in Europe.
It usually occurs 4 to 28 days after vaccination.
