Do I Need Circumcision for My Phimosis Symptoms? - Urology

Phimosis issue

Hello Doctor: My foreskin has a smaller opening when not erect, similar to phimosis.
When not erect, I can fully retract the foreskin behind the glans, and I can also do so when erect.
However, if I keep the foreskin retracted behind the glans for an extended period, about 5-6 hours, there is slight edema around the glans.
Therefore, I usually keep the foreskin covering the glans during daily activities.
When using the restroom, I pull the foreskin back over the glans and return it to its original position afterward.
By the end of the day, the glans tends to have an odor.
Given this situation, do I need a circumcision?

Hello: Your condition seems to involve edema, and it might still feel a bit tight.
If surgery is an option, it would be the quickest way to address the issue, including the odor problem.
As for the odor, it can likely be eliminated through cleaning.
If you prefer not to have surgery, you could consult a physician to see if there is a possibility of using a topical ointment to alleviate the tightness of the skin.
Wishing you good health.

When considering whether circumcision is necessary for phimosis symptoms, it is essential to evaluate both the physical aspects and the potential impact on your quality of life. Phimosis is a condition where the foreskin cannot be easily retracted over the glans (head) of the penis. In your case, it appears that you can retract the foreskin fully when flaccid and erect, which is a positive sign. However, the symptoms you describe, such as mild swelling after prolonged retraction and odor, indicate that there may be some underlying issues that need to be addressed.

First, let's discuss the symptoms you are experiencing. The mild swelling after keeping the foreskin retracted for several hours suggests that there may be some irritation or inflammation occurring. This could be due to friction or lack of lubrication, which is not uncommon. Additionally, the odor you mentioned could be related to the accumulation of smegma, a natural substance that can build up under the foreskin if it is not cleaned regularly. This is particularly common in individuals who have a tighter foreskin or phimosis.

In terms of treatment options, circumcision is one approach, but it is not the only one. Many men with phimosis can manage their symptoms through conservative measures. These may include:
1. Improved Hygiene: Regular cleaning of the area under the foreskin can help prevent the buildup of smegma and reduce odor. It is important to gently retract the foreskin during bathing to clean the glans and the inner foreskin.

2. Topical Treatments: In some cases, topical corticosteroids can be prescribed to help loosen the foreskin and reduce inflammation. This treatment can be effective in making retraction easier and less painful.

3. Stretching Exercises: Gradually stretching the foreskin can sometimes help alleviate phimosis. This should be done gently and with care to avoid causing pain or injury.

4. Consultation with a Specialist: If conservative measures do not alleviate your symptoms, it may be beneficial to consult a urologist. They can provide a thorough evaluation and discuss the potential benefits and risks of circumcision versus other treatment options.

Circumcision is typically recommended when phimosis causes recurrent infections, significant pain, or other complications. If your symptoms are manageable and do not significantly impact your daily life, you may not need to pursue circumcision immediately. However, if you experience recurrent infections, pain, or other complications, circumcision may be a more definitive solution.

In summary, while circumcision is an option for treating phimosis, it is not the only solution. Given that you can retract the foreskin and your symptoms are manageable, it may be worth trying conservative measures first. If these do not provide relief or if your symptoms worsen, discussing circumcision with a healthcare provider would be a prudent next step. Always prioritize open communication with your healthcare provider to ensure that you receive the most appropriate care for your situation.
