Do I need a circumcision?

When I was a child, my parents did not take me for a circumcision, but now, when I have an erection, I cannot retract the foreskin behind the glans, although I can do so when not erect.
I usually clean myself while showering.
Am I experiencing phimosis, and do I need to undergo circumcision?

Phimosis is a condition where the foreskin cannot be retracted over the glans (the head of the penis). It can be classified into two types: physiological phimosis, which is common in infants and young boys and typically resolves on its own as they grow older, and pathological phimosis, which occurs when the foreskin becomes tight due to scarring or other medical conditions.
In your case, since you mentioned that you can retract the foreskin when not erect but cannot do so during an erection, this suggests that you may have a mild form of phimosis. It is important to note that some degree of tightness is normal, especially during puberty, as the skin and tissues are still developing. However, if you experience discomfort, pain, or difficulty during sexual activity, it may warrant further evaluation.

Hygiene is a significant concern with phimosis. If the foreskin cannot be retracted, it can lead to the accumulation of smegma (a combination of dead skin cells and oils) under the foreskin, which can cause irritation, inflammation, or infections. Regular cleaning is essential, and it seems you are already practicing good hygiene by washing regularly. However, if you notice any signs of infection, such as redness, swelling, or discharge, it is crucial to seek medical advice.

Regarding the need for circumcision, it is not always necessary. Many men with phimosis can manage their condition without surgery. Here are some considerations:
1. Conservative Management: If you are not experiencing any pain or recurrent infections, your healthcare provider may recommend a conservative approach. This could include gentle stretching exercises of the foreskin or the use of topical corticosteroids to help loosen the skin.

2. Circumcision: If conservative measures do not work, or if you have recurrent infections or significant discomfort, circumcision may be recommended. This surgical procedure involves the removal of the foreskin and can provide a permanent solution to phimosis. It is generally considered safe and has a low complication rate.

3. Consultation with a Specialist: It is advisable to consult a urologist or a healthcare provider specializing in men's health. They can assess your specific situation, discuss the potential benefits and risks of circumcision, and help you make an informed decision.

4. Personal Preference and Cultural Factors: Some individuals choose circumcision for personal, cultural, or religious reasons. It is essential to consider your values and preferences when making this decision.

In summary, while you may have a form of phimosis, whether or not you need circumcision depends on your symptoms and personal circumstances. If you are experiencing discomfort or have concerns about hygiene, it is best to consult with a healthcare professional who can provide tailored advice and treatment options. Regular follow-ups and open communication with your doctor will help ensure that any issues are addressed promptly and effectively.
