Menstruation that occurs earlier than expected and is shorter in duration?

Hello doctor, my menstrual cycle has typically been around 30 days, but it has suddenly become irregular over the past two months: start dates (duration of bleeding) 3/11 (13 days), 4/13 (9 days), 5/13 (10 days), 6/12 (9 days), and then irregular -- 7/8 (10 days), 7/27 (2 days).
I previously consulted a gynecologist who mentioned that my menstrual duration was longer than average, about 10 days, and that since around 2014, my period has averaged 8 to 9 days.
A year ago, I had medication adjustments that reduced it to about 8 days, but I didn't follow up for a while, and it gradually returned to 9 to 10 days.
Now, not only has my period suddenly started 10 days early, but the bleeding on the first night was quite alarming; I went through two extra-long sanitary pads overnight, and the next day I needed a 35 cm pad every 2 hours.
I could clearly feel the bleeding starting with movement.
However, today, on the third day, there are no blood traces at all; there are no signs on the sanitary pads, and even when urinating, there is no blood color.
When I wipe with tissue, there is also no blood color.
Could this be a false period...
or has it already finished? Or is it counted as part of the last period? Should I see a doctor for these symptoms?

More Info

Irregular menstrual cycles can be concerning, especially when there are significant changes in the timing, duration, and flow of your periods. Based on the information you've provided, it appears that your menstrual cycle has become increasingly unpredictable over the past few months, with periods starting earlier than usual and varying in length. This can be attributed to a variety of factors, and it's essential to understand these potential causes and when to seek medical help.

Possible Causes of Irregular Menstrual Cycles
1. Hormonal Imbalances: The menstrual cycle is regulated by hormones, and any disruption in hormone levels can lead to irregularities. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other endocrine issues can cause fluctuations in your cycle.

2. Stress: High levels of stress can impact your hormonal balance and lead to changes in your menstrual cycle. Stress can affect the hypothalamus, which plays a crucial role in regulating the menstrual cycle.

3. Weight Changes: Significant weight loss or gain can also affect your menstrual cycle. Body fat levels influence estrogen production, and drastic changes can lead to irregular periods.

4. Medications: Certain medications, including hormonal contraceptives, can alter your menstrual cycle. If you've had any recent changes in medication, this could be a contributing factor.

5. Age: As women approach perimenopause, which can begin in their 30s or 40s, they may experience irregular cycles due to hormonal fluctuations.

6. Underlying Health Conditions: Conditions such as endometriosis, fibroids, or pelvic inflammatory disease can also lead to changes in menstrual patterns.

When to Seek Help
Given the significant changes in your menstrual cycle, particularly the sudden onset of heavy bleeding and the drastic change in the timing of your periods, it is advisable to consult a healthcare professional. Here are some specific signs that warrant a visit to the doctor:
- Heavy Bleeding: If you are soaking through one or more sanitary pads or tampons every hour for several consecutive hours, this is considered heavy menstrual bleeding (menorrhagia) and should be evaluated by a doctor.

- Prolonged Periods: If your periods last longer than seven days or if you experience frequent cycles that are significantly shorter than your usual cycle, this should be assessed.

- Severe Pain: If you experience severe pain during your menstrual cycle that disrupts your daily activities, this could indicate an underlying condition that needs to be addressed.

- Other Symptoms: If you notice other symptoms such as fatigue, dizziness, or shortness of breath, especially in conjunction with heavy bleeding, seek medical attention promptly.

Conclusion
In summary, while irregular menstrual cycles can be common and may not always indicate a serious problem, the changes you've described—particularly the heavy bleeding and significant alterations in cycle timing—suggest that it would be prudent to consult a healthcare provider. They can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies to rule out any structural issues. Early intervention can help manage any underlying conditions and restore your menstrual cycle to a more regular pattern.
