Morning Hand Pain: Stiffness and Discomfort in Pinky Joints - Neurology

In the morning upon waking, there is pain, stiffness, and tightness in the joints where the little fingers connect to the palms?

Hello, Doctor.
Since mid-June of this year, I have been experiencing tightness and pain in the joints connecting the little fingers to the palms of both hands every morning upon waking.
After making a fist and releasing it several times, the symptoms gradually improve.
This has been ongoing for over a month.
I have seen a family physician and a rheumatologist, and blood tests have shown no issues with my immune system, ruling out rheumatoid arthritis (with test results at 0).
I have taken pain relievers and anti-inflammatory medications, but there has been no improvement.
Every morning, I still feel tightness and pain on the outer sides of my palms.
I am unsure which specialist I should consult to address my symptoms effectively.

Hello, Miss: Your symptoms may suggest bilateral ulnar nerve compression.
It could also be caused by thoracic nerve root compression.
If the nerve compression leads to peripheral neuropathy, the following characteristics may be present: 1.
Pain with a burning or electric shock-like quality.
2.
Symptoms with a specific dermatomal distribution, which can be triggered by touch or pressure.
3.
Accompanied by muscle soreness and weakness.
I recommend that you visit a neurology clinic for examination and follow-up.
Please ensure to practice self-protection, maintain cleanliness and disinfection, and adhere to new preventive health measures before and after your medical visit.
Thank you for your inquiry, and feel free to discuss any related questions! Wishing you good health, safety, and happiness.

Morning hand pain, particularly stiffness and discomfort in the pinky joints, can be a frustrating and concerning issue. Given your symptoms, which have persisted for over a month, it is essential to consider various potential causes and treatment options.

The discomfort you experience upon waking, particularly in the joints connecting your pinky fingers to your palms, may be attributed to several factors. One common cause is a condition known as "morning stiffness," which is often associated with inflammatory arthritis, but since you have ruled out rheumatoid arthritis through blood tests, we should explore other possibilities.

1. Tendinitis or Tenosynovitis: Inflammation of the tendons or the sheath surrounding them can lead to pain and stiffness, especially after periods of inactivity, such as overnight. This condition can be exacerbated by repetitive motions or strain on the hands.

2. Osteoarthritis: Although more common in older adults, osteoarthritis can occur in younger individuals, particularly if there is a history of joint injuries or overuse. This degenerative joint disease can cause stiffness and pain, especially in the morning.

3. Carpal Tunnel Syndrome: While this condition primarily affects the wrist, it can lead to symptoms in the fingers, including pain and stiffness. It is caused by compression of the median nerve and can be exacerbated by certain activities or positions during sleep.

4. De Quervain's Tenosynovitis: This condition affects the tendons on the thumb side of the wrist and can cause pain that radiates into the fingers, particularly when gripping or making a fist.

5. Other Conditions: Conditions such as Dupuytren's contracture, which involves thickening of the tissue under the skin of the palm, can also lead to discomfort and stiffness in the fingers.

Given that you have already consulted with a family physician and a rheumatologist, and have undergone blood tests that returned normal results, the next step would be to consult with an orthopedic specialist or a hand surgeon. They can perform a thorough physical examination, assess your range of motion, and possibly recommend imaging studies, such as X-rays or an MRI, to evaluate the condition of your joints and surrounding tissues.

In the meantime, there are several self-care strategies you can implement to alleviate your symptoms:
- Gentle Stretching: Before getting out of bed, try gently stretching your fingers and hands to improve circulation and reduce stiffness.

- Warm Compresses: Applying warmth to your hands before use can help relax the muscles and tendons, potentially easing discomfort.

- Activity Modification: If certain activities exacerbate your symptoms, consider modifying them or taking breaks to reduce strain on your hands.

- Splinting: Using a splint or brace during the night may help keep your joints in a neutral position and reduce strain while you sleep.

If your symptoms persist despite these measures, further evaluation and possibly physical therapy may be warranted. A physical therapist can provide targeted exercises to strengthen the muscles around your joints and improve flexibility, which may alleviate your morning stiffness and discomfort.

In summary, while your symptoms are concerning, they are not uncommon and can often be managed effectively with the right approach. Consulting with an orthopedic specialist will provide you with a clearer diagnosis and a tailored treatment plan to help you regain comfort and function in your hands.
