Stiffness and swelling in the finger joints?

Hello Doctor: Last year in late autumn, I noticed stiffness in my finger joints upon waking up, but it was mild, so I didn't pay much attention to it.
Recently, however, the frequency has increased, and almost every day my fingers feel stiff and swollen, with varying degrees of severity, easing up by noon or afternoon.
In May and June, I was awakened by pain in my right index finger, which was quite swollen, and it took until the afternoon for it to improve after applying ice.
From March to July, my work required me to stare at the computer and type continuously, and I wonder if this is related to the symptoms mentioned above.
In March of this year, I had a health check-up, and while my uric acid levels were not tested, I had a positive urine test for blood with two plus signs.
I also noticed increased hair loss last year, but since I had a major exam in January, I'm unsure if it was due to stress or related to the joint swelling.
Additionally, I have experienced chest tightness for several years, and during that time, I had blood tests done at a clinic, where the doctor mentioned my allergy levels were elevated.
However, I have not experienced chest tightness recently, and I am uncertain if this is related to the other symptoms.
I have researched a lot online and am really worried it could be rheumatoid arthritis or another immune system issue, as I do not want my joints to deform or to be on medication for life.
I am eager to go to the hospital for an examination, but due to the pandemic and currently being in the northern part of the country, I feel apprehensive about going to the hospital at this time.
Currently, I am keeping a daily record of my diet (I read that certain foods can trigger symptoms in rheumatoid arthritis patients), but beyond that, I am unsure what to do.
Thank you, Doctor, for your response, and I appreciate this platform for allowing those with concerns to ask questions.

Hello, experiencing stiffness in the finger joints upon waking, pain in the index finger, swelling in the fingers, positive urine occult blood test, and hair loss should not be ignored, but there is no need to panic.
Please seek medical evaluation, as there are many potential causes for morning stiffness that cannot be determined from the description alone.
Thank you for your inquiry.

Understanding stiff and swollen finger joints can be a complex issue, especially when considering the potential underlying causes and the implications for your overall health and daily activities. Based on your description, it seems that you are experiencing increasing stiffness and swelling in your finger joints, particularly in the morning, which improves throughout the day. This pattern can be indicative of several conditions, including inflammatory arthritis, osteoarthritis, or even repetitive strain injuries related to your computer work.

One of the most common causes of morning stiffness in the joints is rheumatoid arthritis (RA), an autoimmune condition that primarily affects the joints, leading to inflammation, pain, and swelling. RA typically presents symmetrically, meaning if one hand is affected, the other is likely to be as well. However, it can also manifest in a more localized manner. Given your concerns about potential joint deformities and the desire to avoid long-term medication, it is crucial to seek a definitive diagnosis from a healthcare professional.

Another possibility is osteoarthritis, which is characterized by the wear and tear of cartilage in the joints. This condition often leads to stiffness after periods of inactivity, such as sleeping, and can worsen with repetitive activities, such as typing or using a computer. The fact that your symptoms have been exacerbated by your work environment suggests that repetitive strain could be a contributing factor.

Your mention of a positive urine test for blood and the increased hair loss could indicate other systemic issues that may or may not be related to your joint symptoms. For instance, conditions like lupus or other autoimmune disorders can present with joint pain, hair loss, and other systemic symptoms. The elevated allergy index you mentioned could also be a sign of an underlying inflammatory process.

Given the complexity of your symptoms and the potential for overlapping conditions, it is advisable to consult with a rheumatologist or an orthopedic specialist. They can conduct a thorough evaluation, including blood tests for inflammatory markers, uric acid levels, and possibly imaging studies to assess the condition of your joints. This will help rule out conditions like rheumatoid arthritis, gout, or other inflammatory arthritides.

In the meantime, there are several strategies you can implement to manage your symptoms. Regular gentle stretching and strengthening exercises can help maintain joint mobility and reduce stiffness. Ergonomic adjustments to your workstation may also alleviate strain on your hands and wrists. Consider using a wrist support or ergonomic keyboard to minimize discomfort while typing.

Additionally, maintaining a healthy diet rich in anti-inflammatory foods, such as omega-3 fatty acids (found in fish), fruits, vegetables, and whole grains, may help reduce inflammation. Staying hydrated and managing stress through relaxation techniques can also be beneficial, as stress can exacerbate inflammatory responses in the body.

If your symptoms persist or worsen, do not hesitate to seek medical attention, even amidst concerns about the pandemic. Many healthcare facilities have implemented safety measures to protect patients, and telemedicine options may also be available for initial consultations.

In summary, while your symptoms may not currently be severely impacting your daily life, they warrant further investigation to prevent potential long-term consequences. Early diagnosis and intervention can significantly improve outcomes and quality of life, allowing you to continue your activities without the fear of debilitating joint issues.
