Discomfort in the lower left abdomen?

Hello Dr.
Chen,
I would like to ask a question.
In recent years, I have been experiencing a feeling of heaviness and discomfort in my lower left abdomen during the mid-cycle of my menstrual period (there was one instance a week before my period).
This discomfort usually occurs at night, but it disappears after a night's sleep.
I have undergone a pelvic ultrasound and a transvaginal ultrasound, and both showed no abnormalities in my uterus or ovaries.

Once, while experiencing this discomfort, I rushed to see a gynecologist, and it turned out that I was ovulating from my right ovary, not the left as I had expected.
Why is it that I feel discomfort on the left side when the ovulation is occurring on the right? This has always been the case; I have never felt discomfort on the right side.
The discomfort even extends to when I have a bowel movement, and it feels like a pulling pain in the fascia (only in the lower left abdomen).

I have also observed my vaginal discharge, which is clear, thin, and stretchy, indicating that I am indeed in my ovulation phase.
However, there was one instance when the discomfort in my lower left abdomen occurred a week before my period, which wouldn't be classified as ovulation discomfort.
Based on my description, could this be related to a gynecological condition, or should I consult a gastroenterologist? I am quite worried.

Understanding left lower abdominal discomfort can be quite complex, especially when symptoms vary in intensity and timing. Based on your description, it seems that you experience discomfort in the left lower abdomen primarily during the mid-cycle of your menstrual cycle, which aligns with ovulation. However, the fact that you have had discomfort even a week before your menstruation suggests that there could be multiple factors at play.

Firstly, it's important to recognize that the left lower abdomen houses several organs, including parts of the intestines, the left ovary, and the left kidney. Discomfort in this area can arise from various sources, including gastrointestinal issues, gynecological conditions, or even musculoskeletal problems.

1. Gynecological Factors: Given that you have noted discomfort correlating with your menstrual cycle, it is plausible that this could be related to ovulation pain (mittelschmerz). This pain typically occurs on one side of the abdomen, depending on which ovary is releasing an egg. In your case, it seems that the discomfort is consistently on the left side, even when the right ovary is the one ovulating. This could be due to referred pain, where pain is perceived in a different location than its source. Additionally, conditions such as ovarian cysts or endometriosis could also cause localized pain, particularly if there are changes in the hormonal environment during your cycle.

2. Gastrointestinal Factors: The left lower quadrant of the abdomen is also home to parts of the colon, and issues such as diverticulitis, irritable bowel syndrome (IBS), or even constipation could lead to discomfort in this area. You mentioned that the discomfort sometimes feels like a pulling sensation, which could suggest that it is related to bowel movements or gas. If you experience changes in bowel habits or other gastrointestinal symptoms, it may be worthwhile to consult a gastroenterologist.

3. Musculoskeletal Issues: Sometimes, discomfort in the abdomen can stem from musculoskeletal problems, such as muscle strain or issues with the abdominal wall. If you have been engaging in activities that strain your core muscles, this could lead to discomfort that mimics gastrointestinal or gynecological pain.

4. Hormonal Fluctuations: Hormonal changes throughout your menstrual cycle can also affect how you perceive pain. For instance, increased levels of prostaglandins during menstruation can lead to cramping and discomfort, which might explain the pain you feel a week before your period.

Given that you have already undergone ultrasound examinations of the uterus and ovaries, and these have shown no abnormalities, it may be beneficial to keep a detailed symptom diary. This diary should include the timing of your symptoms, their intensity, any associated activities, and any other symptoms you may experience. This information can be invaluable for your healthcare provider in determining the underlying cause of your discomfort.

In conclusion, while your symptoms may have a gynecological basis, it is essential to consider other potential causes, including gastrointestinal and musculoskeletal factors. If your symptoms persist or worsen, I recommend consulting with both a gynecologist and a gastroenterologist to ensure a comprehensive evaluation. They may suggest further imaging studies, such as a CT scan or MRI, or other diagnostic tests to pinpoint the cause of your discomfort. Remember, your health is paramount, and seeking answers is a crucial step in managing your symptoms effectively.
