Testicular issues
Hello Doctor, I recently noticed a soft lump on the lower right side of my right testicle.
As I palpated it, I found that it extends along the back of the testicle up to the upper part.
Should I assume this is the epididymis? However, the epididymis on the back of my left testicle does not feel the same; it is actually quite difficult to palpate, while the epididymis on the front side of the left testicle is relatively easy to feel (in contrast, the front side of the right testicle is not easy to palpate).
Could you explain why this discrepancy occurs? Is it possible that the positions of my testicles are reversed (is this torsion? But I don't feel any pain)? Or is it normal for the epididymides of the two testicles to not be identical? Thank you, Doctor.

It does sound like it could be the epididymis.
Generally, it is not easy to palpate, and the two sides may not be completely symmetrical.
If there are any doubts, it is best to consult a urologist for an examination.

It sounds like you have some concerns about the anatomy of your testicles and the epididymis, particularly regarding the differences you’ve noticed between the right and left sides. Let’s break this down and provide some clarity.
First, it’s important to understand the anatomy of the testicles and the epididymis. The epididymis is a coiled tube located at the back of each testicle, and it plays a crucial role in the maturation and storage of sperm. It is normal for the epididymis to have some variation in size and shape between the two sides. This can be due to a variety of factors, including individual anatomical differences, hormonal influences, and even the position of the testicles in the scrotum.
In your case, you mentioned that the right epididymis feels more prominent and easier to palpate than the left. This is not uncommon, as anatomical asymmetry is a normal occurrence in many individuals. The epididymis on one side may be more developed or simply positioned differently, which can make it feel more pronounced. Additionally, the texture and firmness of the epididymis can vary, and this can also contribute to the differences you are feeling.
You also expressed concern about whether this could indicate a condition such as testicular torsion. Testicular torsion is a medical emergency that occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle. However, torsion is typically associated with severe pain, swelling, and sometimes nausea. Since you mentioned that you are not experiencing any pain, it is less likely that you are dealing with torsion.
It’s also worth noting that the epididymis can sometimes be more difficult to palpate on one side due to its position or the surrounding tissue. Factors such as body position, muscle tension, and even the temperature of the scrotum can affect how easily you can feel the epididymis.
If you are still concerned about the differences you are noticing, it would be a good idea to consult with a healthcare professional, such as a urologist. They can perform a physical examination and, if necessary, imaging studies like an ultrasound to ensure that everything is normal.
In summary, while it is normal for the epididymis to have some asymmetry, if you notice any changes in size, shape, or if you experience pain or discomfort, it is advisable to seek medical attention. Regular self-examinations of the testicles can help you become familiar with your anatomy, making it easier to notice any changes that may occur over time. Remember, your health and peace of mind are important, so don’t hesitate to reach out to a medical professional if you have concerns.
