COVID-19 Vaccine: Should You Pause Antihistamines and Anxiety Meds? - Internal Medicine

Vaccine medication consultation

Hello: I have been taking the antihistamine Levocetirizine (Xyzal) every night due to nasal allergies.
Should I stop taking this medication before and after receiving the COVID-19 vaccine? If so, how many days should I stop it before and after? Additionally, I have been taking the anti-anxiety medication Lexotan (Bromazepam) 3mg every night for autonomic nervous system dysregulation.
Can I continue taking this medication as usual when I get the COVID-19 vaccine? I have been on it for a year and am concerned about withdrawal symptoms if I stop suddenly.
Thank you!

DAMY, 40~49 year old female. Ask Date: 2021/08/11

Dr. Hong Yuanbin reply Internal Medicine

Hello, antihistamines and autonomic nervous system medications do not affect the efficacy of vaccines, so it is fine to continue taking them.
Thank you for your question.

When considering the administration of the COVID-19 vaccine, many individuals have questions regarding the use of medications, particularly antihistamines and anxiety medications. In your case, you are taking Levocetirizine for nasal allergies and Lexotan (bromazepam) for anxiety.
Firstly, it is important to note that both Levocetirizine and Lexotan do not interfere with the efficacy of the COVID-19 vaccine. Therefore, you do not need to pause or stop taking Levocetirizine before or after receiving the vaccine. There is no specific requirement to discontinue antihistamines prior to vaccination, and you can continue taking it as prescribed. The same applies to Lexotan; you can continue your medication without interruption.
The concern about stopping Lexotan suddenly is valid, as abrupt discontinuation of benzodiazepines can lead to withdrawal symptoms, including increased anxiety, insomnia, and even seizures in severe cases. It is advisable to maintain your regular dosage of Lexotan around the time of vaccination to avoid any potential withdrawal effects. If you have concerns about the long-term use of Lexotan or wish to taper off, it is best to discuss this with your prescribing physician, who can provide a safe and effective tapering plan.

In terms of timing, there is no need to stop either medication days before or after the vaccination. You can take your medications as usual on the day of the vaccine and continue them afterward. This approach ensures that you manage your allergy symptoms and anxiety effectively while also receiving the vaccine.

It is also worth mentioning that anxiety about vaccination is common, especially given the ongoing concerns surrounding COVID-19. If you find that your anxiety levels increase around the time of vaccination, consider employing relaxation techniques such as deep breathing, mindfulness, or meditation, which can help mitigate anxiety without interfering with your medications.

In summary, you can safely continue taking Levocetirizine and Lexotan without interruption when receiving the COVID-19 vaccine. If you have further concerns or experience any unusual symptoms after vaccination, do not hesitate to reach out to your healthcare provider for guidance. Your health and well-being are paramount, and maintaining open communication with your healthcare team is essential in managing both your allergies and anxiety effectively.
