Breast issues

Hello doctor, I recently noticed a bruise under my left breast that appeared suddenly.
It hurts a little when pressed (but not very much).
At first, it felt itchy, but after applying some medication, the itchiness went away.
However, the bruise has remained for about three days now.
My daily life and diet are normal.
What could this bruise be? What can I do to help it heal? Thank you.

Wo mei you ming zi, 40~49 year old female. Ask Date: 2021/08/26

Dr. Chen Jiaming reply Surgery

It is recommended to directly consult a breast surgeon for further ultrasound examination to confirm whether there are any other lesions present in the subcutaneous tissue.

Breast bruising can be concerning, especially when it appears suddenly and without a clear cause. The presence of a bruise, or contusion, on the breast can arise from various factors, and understanding these can help in managing the situation effectively.

Causes of Breast Bruising
1. Trauma or Injury: The most common cause of a bruise is trauma. This can occur from a direct impact, such as bumping into something, or even from vigorous activities like exercise. Sometimes, the injury may not be remembered, especially if it was minor.

2. Hormonal Changes: Hormonal fluctuations, particularly those related to the menstrual cycle, can make breast tissue more sensitive and prone to bruising. During certain phases of the cycle, breasts may swell and become more vascular, which can lead to increased susceptibility to bruising.

3. Blood Disorders: Conditions that affect blood clotting, such as thrombocytopenia or hemophilia, can lead to easy bruising. If you notice frequent bruising in other areas of your body, it may be worth consulting a healthcare provider.

4. Medications: Certain medications, especially blood thinners (like aspirin or warfarin), can increase the likelihood of bruising. If you are taking any medications, it’s important to consider their side effects.

5. Skin Conditions: Sometimes, skin conditions or infections can cause discoloration that may resemble a bruise. If the area is itchy or has other symptoms, this could be a possibility.

Treatment Options
1. Rest and Avoidance: If the bruise is due to an injury, it’s important to avoid further trauma to the area. Resting the affected side can help in the healing process.

2. Cold Compress: Applying a cold compress to the bruise for the first 24-48 hours can help reduce swelling and pain. After that period, warm compresses can promote blood flow and healing.

3. Topical Treatments: Over-the-counter creams or gels containing arnica or vitamin K may help in reducing the appearance of bruises. However, it’s essential to ensure that these products do not irritate the skin.

4. Pain Management: If there is discomfort, over-the-counter pain relievers like acetaminophen or ibuprofen can be used, but it’s advisable to avoid aspirin as it can exacerbate bruising.

5. Monitoring: Keep an eye on the bruise. If it does not improve within a week, or if you notice other concerning symptoms (like increased pain, swelling, or changes in the breast), it’s crucial to seek medical advice.

When to Seek Medical Attention
If the bruise persists beyond a week, becomes increasingly painful, or if you experience other symptoms such as fever, unusual discharge, or changes in breast shape or texture, it’s important to consult a healthcare professional. They may perform a physical examination and, if necessary, imaging studies to rule out any underlying issues.

In conclusion, while a bruise on the breast can be alarming, it is often a benign condition resulting from minor trauma or hormonal changes. By taking appropriate care and monitoring the situation, most bruises will heal without complications. However, staying vigilant and seeking medical advice when necessary is always the best approach to ensure breast health.
