Glans issues
Hello doctor, in the past few days, a small area at the opening of my foreskin has suddenly become very dry and hard, and it has cracked, similar to chapped lips.
It hurts as well, especially when I pull it back.
I'm not sure if this is balanitis.
Can I apply lotion or petroleum jelly? I do not have phimosis and I clean it thoroughly every day.
Thank you.

Dr. Du Shixi reply Urology
Hello: If there is a crack, there may be an injury.
Do not use lotion or petroleum jelly; you should seek medical attention to check for any injuries or infections.
Wishing you good health.
Currently, keep the area clean and avoid moisture.

More Info
Understanding glans issues, particularly conditions like balanitis or other related disorders, is essential for proper management and treatment. The symptoms you describe, such as dryness, cracking, and pain upon retraction of the foreskin, can indeed be indicative of a condition affecting the glans or the surrounding area.
Symptoms
The symptoms of glans issues can vary widely but often include:
- Dryness and Cracking: As you mentioned, a dry and cracked appearance can occur, resembling chapped lips. This can be uncomfortable and painful, especially when the area is stretched.
- Redness and Inflammation: The glans may appear red or inflamed, which is a sign of irritation or infection.
- Pain or Discomfort: Pain during retraction of the foreskin or during sexual activity can be a significant symptom.
- Itching or Burning Sensation: These sensations can accompany other symptoms and may indicate an underlying infection or irritation.
Causes
Several factors can contribute to glans issues, including:
- Infections: Bacterial or fungal infections, such as candidiasis (yeast infection), can lead to symptoms like those you are experiencing. Balanitis is a common term used to describe inflammation of the glans, often due to infections.
- Irritation: This can be caused by soaps, lotions, or other personal care products that may not be suitable for sensitive areas. Friction from clothing or sexual activity can also lead to irritation.
- Dermatological Conditions: Conditions such as psoriasis or eczema can affect the glans and surrounding skin, leading to dryness and cracking.
- Poor Hygiene: While you mentioned maintaining cleanliness, inadequate hygiene can lead to the accumulation of smegma, which can irritate the glans.
Treatment Options
Treatment for glans issues typically focuses on addressing the underlying cause and alleviating symptoms:
1. Topical Treatments: Applying a gentle, fragrance-free moisturizer such as petroleum jelly (Vaseline) or a hypoallergenic lotion can help soothe dryness and promote healing. However, it is essential to avoid products with fragrances or irritants.
2. Antifungal or Antibacterial Creams: If an infection is suspected, a healthcare provider may recommend topical antifungal or antibacterial creams.
3. Avoiding Irritants: Discontinue the use of any soaps, lotions, or other products that may be causing irritation. Opt for mild, unscented products designed for sensitive skin.
4. Proper Hygiene: Continue to maintain good hygiene by gently cleaning the area with warm water and avoiding harsh soaps.
5. Consulting a Healthcare Provider: If symptoms persist or worsen, it is crucial to seek medical advice. A healthcare provider can perform a physical examination, possibly conduct tests, and provide a more accurate diagnosis and tailored treatment plan.
Conclusion
While it is possible that your symptoms may be related to a mild case of balanitis or another irritation, it is essential to monitor the situation closely. If you notice any worsening of symptoms, such as increased pain, swelling, or discharge, it is advisable to consult a healthcare professional for a thorough evaluation. Early intervention can help prevent complications and ensure appropriate care. Remember, self-diagnosis can be misleading, and professional guidance is always the best course of action when dealing with health concerns.
