Wound after a motorcycle accident?

Hello Doctor, I had a motorcycle accident on July 23, and since then, I have severe abrasions on my right foot with four wounds.
I went to the hospital for a diagnosis, and the doctor assessed the abrasions on my right foot as equivalent to a second-degree burn.
Currently, there is only about one centimeter left to heal, but for the past two weeks, I have felt tightness in the affected area.
Could this be due to poor blood circulation in the newly formed skin? My foot is also swollen.
I am very worried that the wounds are not being cared for properly.
Is it possible to avoid the wounds and gently massage the areas where new skin has formed? Additionally, one of the wounds has healed, but there is a depression in the middle, and it feels like the tissue hasn't fully developed yet.
When I press on it, it still feels a bit hard.
Should I press on it daily to help soften the scar? Can I apply silicone gel or scar gel only after the wounds have completely healed? Thank you.

Hello! High-speed grinding wounds are similar to burns and may also have issues with contractures.
You can go to a rehabilitation clinic for therapy.
Once the wound has healed and the skin has grown over, the tissue will not increase further.
The remaining concern is to reduce scarring, so please apply scar gel.
Take care! Changhua Hospital cares about you!

After a motorcycle accident, managing wound healing is crucial for both physical recovery and minimizing long-term complications. Based on your description, it sounds like you are dealing with significant abrasions on your right leg, which have been classified as second-degree burns. Here are some tips and concerns regarding your situation:

Understanding Wound Healing
1. Wound Healing Process: The healing process involves several stages: hemostasis (stopping the bleeding), inflammation, proliferation (tissue formation), and remodeling (maturation). The tightness you feel in the area may be due to the proliferation phase, where new tissue is forming. This can sometimes cause discomfort or a sensation of tightness as the skin stretches.

2. Blood Circulation: If you notice swelling in your foot, it could indicate that blood circulation is affected, especially if you are keeping your leg elevated or immobile for extended periods. Gentle movement can help promote circulation, but it’s essential to avoid putting too much strain on the healing skin.

Care for the Wound
1. Avoiding Tension on the Wound: It’s generally advisable to avoid pulling or stretching the new skin. Instead, focus on gentle movements that do not put direct pressure on the wound. If you feel the need to massage the area, do so very gently and only if it does not cause pain.

2. Managing Scarring: For the area that has healed but still has a depression, it’s important to monitor it. Pressing on it can help soften the tissue, but be cautious not to apply excessive pressure. Once the wound is fully healed, you can start using silicone gel sheets or scar creams to help minimize scarring. These products are most effective when applied to fully healed skin.

3. Monitoring for Infection: Keep an eye on the wounds for any signs of infection, such as increased redness, warmth, swelling, or discharge. If you notice any of these symptoms, it’s crucial to seek medical attention promptly.

When to Seek Medical Attention
- If the tightness and swelling do not improve or worsen, or if you experience increased pain, it’s essential to consult your healthcare provider. They may recommend physical therapy or other interventions to aid in recovery.

- If the area with the depression does not improve over time or if you have concerns about the healing process, follow up with your doctor. They may suggest imaging studies or further evaluation to ensure proper healing.

General Recommendations
- Hydration and Nutrition: Ensure you are well-hydrated and consuming a balanced diet rich in vitamins and minerals, particularly vitamin C and protein, which are vital for skin healing.

- Avoiding Sun Exposure: Protect healing skin from sun exposure, as UV rays can darken scars and hinder healing.

- Follow-Up Appointments: Keep all follow-up appointments with your healthcare provider to monitor your healing progress and address any concerns.

In conclusion, while it’s normal to have concerns about wound healing after an accident, following proper care guidelines and maintaining open communication with your healthcare provider will help ensure a smoother recovery. If you have any doubts or if your symptoms change, don’t hesitate to seek medical advice. Your health and recovery are paramount.
