Wound issues after a car accident?

Hello, doctor.
After I fell off my bike on December 10, my knee went from a bleeding wound to a soft pink area with no open wound.
However, I still experience a tingling sensation when bending quickly (squatting), walking, or applying slight pressure to the injured area.
In a resting state, I no longer feel any sensation when touching the injured area.
I initially thought it was due to the cold weather, and I applied petroleum jelly, but there has been no improvement.
I would like to ask if this healing process is normal.

YY, 20~29 year old female. Ask Date: 2022/12/30

Dr. Chen Bingze reply Surgery

Hello, it sounds like the wound should be okay, but you may still experience some tingling and numbness.
This could be due to some peripheral nerve damage, which should improve over time.

More Info

After a motorcycle accident, it is common to experience various stages of wound healing, especially when dealing with injuries such as abrasions or lacerations on the knee. Based on your description, it seems that your wound has progressed from an open bleeding state to a pink, healing area, which is a positive sign indicating that the healing process is underway. However, the sensations you are experiencing—such as tingling or a prickly feeling when bending your knee or applying pressure—can be concerning and warrant further discussion.

Understanding Wound Healing
Wound healing is a complex biological process that occurs in several stages: hemostasis, inflammation, proliferation, and remodeling. Initially, when the skin is injured, the body works to stop the bleeding (hemostasis). Following this, an inflammatory response occurs, which is characterized by redness, swelling, and pain as the body sends white blood cells to the site to prevent infection. The proliferation phase involves the formation of new tissue, and finally, the remodeling phase can last for months to years, where the new tissue matures and strengthens.

In your case, the transition from a bleeding wound to a pink area suggests that you are moving from the inflammatory phase into the proliferation phase. The pink color indicates new blood vessel formation and the presence of granulation tissue, which is essential for healing.

Sensations and Symptoms
The tingling or prickly sensations you describe could be attributed to several factors:
1. Nerve Regeneration: If the injury involved any nerve damage, the sensations you feel could be a sign of nerve regeneration. As nerves heal, they can sometimes send mixed signals to the brain, resulting in unusual sensations.

2. Scar Tissue Formation: As the wound heals, scar tissue forms, which can sometimes lead to altered sensations in the area. This is particularly true if the scar tissue is tight or if it affects nearby nerves.

3. Inflammation: Even though the wound appears to be healing, there may still be some residual inflammation that can cause discomfort or unusual sensations when the area is manipulated.

4. Cold Weather Effects: You mentioned that you thought the cold weather might be affecting your healing. Cold temperatures can indeed affect circulation and may lead to increased sensitivity in previously injured areas.

Recommendations
1. Monitor the Healing Process: Continue to observe the wound for any signs of infection, such as increased redness, swelling, warmth, or discharge. If these symptoms occur, seek medical attention.

2. Gentle Movement: While it is important to keep the knee mobile to prevent stiffness, avoid any activities that cause significant pain. Gradually increase your range of motion as tolerated.

3. Moisturization: While you have been using Vaseline, consider using a specialized wound healing ointment or silicone gel, which can help with scar formation and keep the area hydrated.

4. Consult a Healthcare Professional: If the tingling sensations persist or worsen, or if you have concerns about the healing process, it would be wise to consult with a healthcare professional. They may recommend physical therapy or further evaluation to ensure that there are no underlying issues affecting your recovery.

5. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort as you heal.

In summary, while your wound appears to be healing normally, the sensations you are experiencing are worth monitoring. If you have any concerns or if the symptoms do not improve, do not hesitate to seek medical advice. Proper care and attention to your injury will facilitate a smoother recovery process.
