Bruising on the thigh one week after receiving the AZ vaccine?

Hello doctor, I received my first dose of the AstraZeneca vaccine on September 9th, and today I suddenly noticed a bruise on the left side of my left thigh.
There are no red spots of bleeding, but it is quite painful when pressed.
The bruise is irregular in shape and larger than a one-dollar coin.
I'm not sure if it was caused by a bump, but I'm a bit worried that it might be a symptom of a vaccine-related thrombosis.
This morning, I also experienced abdominal pain and diarrhea, which could possibly be due to the barbecue I had yesterday.
After taking some antacids, the stomach pain has subsided.
I would like to ask the doctor what a thrombosis bruise typically looks like and if my symptoms resemble those of thrombosis.
Which department should I visit for medical attention? Thank you.

Q R, 20~29 year old female. Ask Date: 2021/09/22

Dr. Ye Qianyu reply Family Medicine

Hello, after a thrombosis occurs, the symptoms vary depending on the location.
For instance, if it occurs in the cerebral vessels, it can cause symptoms such as blurred vision and headaches.
If it occurs in the lower limbs, there may be unilateral swelling and pain.
If it occurs in the lungs, it can lead to shortness of breath.
Based on your description, it does not seem to be a thrombosis.
I recommend that you continue to monitor your condition, and if you have any concerns, please consult a cardiovascular surgeon.

After receiving the AstraZeneca (AZ) vaccine, it is not uncommon for individuals to experience various side effects, including bruising. However, it is essential to differentiate between normal post-vaccination reactions and symptoms that may indicate a more serious condition, such as thrombosis.

Bruising can occur for several reasons after vaccination. The injection itself can cause localized trauma to the tissue, leading to bruising, especially if the needle inadvertently punctures a small blood vessel. This type of bruise typically appears as a discolored area on the skin, which may be tender to the touch but generally resolves within a few days to a week.

In your case, you mentioned that you have a bruise on your left thigh that is painful when pressed and is irregularly shaped, larger than a coin. While this could be a simple bruise from the injection, it is prudent to be cautious, especially given your concerns about potential blood clotting issues associated with the AZ vaccine.

Blood clots related to the AZ vaccine can manifest in various ways, but they often present as swelling, pain, and discoloration in the affected area. The bruising associated with a blood clot may not necessarily look different from a regular bruise, but it may be accompanied by other symptoms such as swelling, warmth, and persistent pain. If you experience any of these symptoms, it is crucial to seek medical attention promptly.

Additionally, you mentioned experiencing abdominal pain and diarrhea, which could be unrelated to the vaccine and more likely associated with dietary choices, such as consuming barbecued food. However, gastrointestinal symptoms can also occur after vaccination, so it's essential to monitor these symptoms closely.

Given your concerns about the bruise and the potential for thrombosis, it is advisable to consult a healthcare professional. You should consider visiting a primary care physician or an urgent care clinic first. They can evaluate your symptoms, perform a physical examination, and determine whether further testing is necessary, such as an ultrasound to assess for deep vein thrombosis (DVT) or other imaging studies.

In summary, while bruising after the AZ vaccine can be a normal reaction, it is essential to remain vigilant for any signs of complications. If the bruise worsens, if you develop new symptoms, or if you have any concerns about your health, do not hesitate to seek medical advice. Your health and safety are paramount, and healthcare professionals are equipped to provide the necessary guidance and care.
