Frequent urination and penile discomfort?

Hello doctor, I often feel a soreness in my penis and scrotum when I lie down, especially when lying flat, and I have a strong urge to urinate even though I just went to the bathroom not long ago.
When I do go, I only pass a small amount of urine.
I frequently find myself running to the bathroom but not urinating much, and this has been affecting my sleep quality over time.
I am currently in Australia, where medical and testing costs are relatively high, so I would like to know what might be causing this issue.
Thank you.

Hello: The information provided here is for reference only and cannot replace a physician's examination.
If there have been no unsafe sexual behaviors, the symptoms may be early signs of prostate issues.
Given your young age, consider making lifestyle adjustments, paying attention to your diet, and incorporating exercise.
With some attention over time, improvements may be observed.
Wishing you good health.

More Info

Frequent urination and discomfort in the genital area can be distressing symptoms that significantly affect your quality of life, especially when they interfere with sleep. Based on your description, there are several potential causes for your symptoms, and understanding these can help guide you toward appropriate solutions.

Possible Causes
1. Overactive Bladder (OAB): This condition is characterized by a sudden urge to urinate that may be difficult to control. It can lead to frequent urination, including at night (nocturia). OAB can occur even when the bladder is not full, leading to the sensation of needing to urinate shortly after using the bathroom.

2. Urinary Tract Infection (UTI): Although you mentioned that you recently urinated, a UTI can cause frequent urges to urinate, often accompanied by discomfort or a burning sensation. Even if you do not have other symptoms like fever or back pain, a UTI can still be present.

3. Prostatitis: In men, inflammation of the prostate gland can lead to symptoms such as pelvic pain, discomfort in the genital area, and urinary issues. This condition can be acute or chronic and may require specific treatment.

4. Bladder Irritation: Certain foods, drinks, or medications can irritate the bladder, leading to increased urgency and frequency of urination. Common irritants include caffeine, alcohol, spicy foods, and artificial sweeteners.

5. Nerve Issues: Conditions affecting the nerves that control the bladder can lead to abnormal sensations and urinary frequency. This could be due to diabetes, multiple sclerosis, or other neurological conditions.

6. Psychological Factors: Anxiety and stress can also manifest as urinary frequency. If you find yourself preoccupied with the need to urinate, it may exacerbate the sensation.

Recommendations
1. Keep a Bladder Diary: Documenting your fluid intake, urination frequency, and any associated symptoms can help identify patterns and triggers. This information can be valuable for your healthcare provider.

2. Limit Irritants: Try to identify and reduce the intake of potential bladder irritants. This includes caffeine, alcohol, and spicy foods.

3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can help improve bladder control and reduce urgency.

4. Scheduled Voiding: Establishing a regular schedule for urination can help train your bladder and reduce the frequency of urges.

5. Consult a Healthcare Provider: Given the complexity of your symptoms and the potential for various underlying conditions, it is advisable to consult a healthcare provider. They may recommend tests such as urinalysis, ultrasound, or urodynamic studies to assess bladder function and rule out infections or other issues.

6. Medication: If your symptoms are severe or persistent, medications may be prescribed to help manage overactive bladder or prostatitis, depending on the underlying cause.

Conclusion
While it can be challenging to navigate healthcare costs in Australia, addressing these symptoms is important for your overall well-being. If possible, seek out a urologist or a primary care physician who can provide a thorough evaluation and guide you toward effective treatment options. Early intervention can often lead to better outcomes and improved quality of life.
