Urinary Issues: Frequent Urination and Testicular Pain - Urology

Prostate concerns?

I have been experiencing frequent urination, about once every hour, for the past month.
I visited a clinic for a urinalysis, and the doctor said everything was normal, so I was prescribed some medication.
I noticed slight improvement, but now I have developed other issues: I feel intermittent pain (aching) in my left testicle and the inner side of my left thigh.
My urine flow is weak, and I have to exert effort to urinate more smoothly.
I have had a habit of masturbation for three years, almost daily.
I read some articles that suggest frequent diarrhea could cause this, or could it be related to my cycling routine? I am unsure if this is what has led to my current issues, and it is really troubling me.
Additionally, I tend to get quite anxious.
I would greatly appreciate a detailed explanation.
Thank you!

Based on your description, it is suspected that emotional factors may be causing frequent urination (overactive bladder), and pelvic floor muscle tension may be leading to testicular and inner thigh pain as well as difficulty urinating (often diagnosed as chronic prostatitis).
It is recommended to avoid excessive stress.
Additionally, when cycling, do not ride for too long; taking breaks every 30-60 minutes can help prevent poor blood circulation.
You may also try pelvic floor muscle contraction exercises (Kegel exercises) and pelvic floor sitz baths with heat therapy.

More Info

Frequent urination, testicular pain, and associated urinary issues can be quite distressing and may stem from various underlying conditions. Your symptoms, including the need to urinate every hour, the pain in your left testicle and inner thigh, and the difficulty in urination, warrant a thorough evaluation to determine the root cause.

Possible Causes of Your Symptoms
1. Urinary Tract Infection (UTI): Although your initial urine tests may not have indicated an infection, UTIs can sometimes present with intermittent symptoms. They can cause frequent urination, urgency, and discomfort during urination.
2. Prostatitis: In men, inflammation of the prostate gland (prostatitis) can lead to urinary symptoms such as frequency, urgency, and pain. It can also cause referred pain to the testicles and inner thighs. This condition can be acute or chronic and may require specific antibiotics or anti-inflammatory medications for treatment.

3. Testicular Issues: Pain in the testicles can arise from various conditions, including epididymitis (inflammation of the epididymis), testicular torsion, or even referred pain from the lower urinary tract. Given your symptoms, it would be prudent to have a scrotal ultrasound to rule out any structural issues.

4. Nerve Compression or Irritation: Given your history of cycling, it’s possible that prolonged pressure on the perineal area could lead to nerve irritation, which might manifest as pain in the testicles and inner thigh. This is sometimes referred to as "cyclist's syndrome."
5. Psychological Factors: You mentioned being easily anxious, which can exacerbate physical symptoms. Anxiety can lead to increased muscle tension and may contribute to urinary urgency and discomfort.

6. Bowel Issues: Frequent diarrhea can lead to dehydration and electrolyte imbalances, which might indirectly affect urinary function. Additionally, gastrointestinal issues can sometimes cause referred pain to the pelvic area.

Recommendations for Management
1. Follow-Up with a Specialist: Given the complexity of your symptoms, it may be beneficial to consult a urologist. They can perform a more detailed evaluation, including a physical examination, imaging studies, and possibly a cystoscopy if indicated.

2. Urine Culture: If not already done, a urine culture can help identify any hidden infections that might not show up on routine tests.

3. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage discomfort. However, consult your doctor before starting any new medication.

4. Hydration: Continue to drink plenty of fluids, as this can help flush out any potential irritants in the urinary tract.

5. Modify Activities: If cycling is contributing to your symptoms, consider taking a break or adjusting your bike setup to reduce pressure on the perineal area.

6. Stress Management: Since you mentioned being prone to anxiety, incorporating relaxation techniques such as deep breathing, meditation, or yoga may help alleviate some of the tension that could be contributing to your symptoms.

7. Dietary Considerations: Pay attention to your diet, as certain foods and drinks can irritate the bladder. Caffeine, alcohol, and spicy foods are common culprits.

Conclusion
Your symptoms of frequent urination and testicular pain are concerning and should not be ignored. It’s essential to seek comprehensive medical evaluation to rule out serious conditions and to receive appropriate treatment. Keeping a symptom diary, noting when symptoms occur, their intensity, and any associated activities, can be helpful for your healthcare provider in diagnosing your condition. Don’t hesitate to advocate for yourself during medical visits; if you feel your concerns are not being addressed, it’s perfectly acceptable to seek a second opinion.
