Hearing problems

Hello, doctor.
I would like to ask you a question.
Recently, when I listen to music, I feel that the pitch is either slightly higher or lower than it should be, making the music sound unpleasant and discordant.
I usually have regular exposure to music, so this has been quite troubling for me.
I have a good sense of pitch, so I can easily detect even slight differences.
I'm not sure if this issue is serious and how I can resolve it.
Thank you for your guidance.

Additionally, I would like to mention that I have been undergoing treatment for epilepsy and have been taking medications such as Lamotrigine and Carbamazepine.

When you experience a change in how music sounds—specifically, if it seems to be out of tune or off-key—this can be indicative of several underlying issues, particularly if you have a keen sense of pitch. Given your background in music and your recent medical history, it is essential to consider both auditory and neurological factors that may be contributing to your symptoms.

Firstly, the perception of music sounding "off-key" can be linked to auditory processing issues. The auditory system is complex, involving not just the ears but also the brain's ability to interpret sounds. If there is any disruption in this processing, it can lead to distortions in how we perceive pitch and harmony. This could be due to exposure to loud sounds, which can cause temporary or permanent changes in hearing, known as noise-induced hearing loss. If you have been exposed to high decibel levels, such as from concerts or loud music, this could be a contributing factor.

Moreover, your mention of being on medication for epilepsy, specifically carbamazepine (known as Tegretol), raises another important consideration. Carbamazepine can have side effects that may affect auditory perception, including dizziness or alterations in sensory processing. If you have recently started or adjusted your dosage, this could potentially influence how you perceive sound. It is crucial to discuss these changes with your prescribing physician, as they may need to evaluate whether your current medication regimen is appropriate or if adjustments are necessary.

Additionally, it is worth noting that psychological factors can also play a role in how we perceive music. Stress, anxiety, or even fatigue can alter auditory perception. If you are experiencing any emotional or psychological stressors, this could manifest as a change in how you experience music. Techniques such as mindfulness or relaxation exercises may help mitigate these effects.

Given these considerations, it is advisable to seek a comprehensive evaluation from an audiologist or an ear, nose, and throat (ENT) specialist. They can conduct a thorough hearing assessment, including tests for auditory processing and any potential damage to the auditory system. If necessary, they may refer you for further neurological evaluation to rule out any effects related to your epilepsy treatment.

In the meantime, you might find it beneficial to take breaks from listening to music at high volumes and to engage in activities that promote auditory health, such as listening to music at lower volumes or in quieter environments. Additionally, practicing ear protection in loud settings can help prevent further auditory damage.

In summary, while your experience of music sounding off-key can be concerning, it is essential to approach this issue holistically, considering both auditory and neurological factors. Consulting with healthcare professionals will provide you with the best course of action to address your symptoms and ensure your auditory health is preserved.
