Managing Penile Skin Tears: Solutions for Persistent Issues - Urology

Foreskin tear issue?

In the past, my foreskin covered the glans when not erect.
However, since I started retracting the foreskin below the glans, it has gradually stopped returning to its original position when not erect.
Instead, it has become a layer of skin beneath the glans, forming folds when not erect.
During an erection, these folds stretch out, but each time this happens, I experience tearing and bleeding.
Over the past four years, I have had countless instances of tearing and healing.
Each tear occurs in a ring-like pattern on a section of the penis where the foreskin has been retracted.
Although the wounds are not as large as they were initially, I still feel pain from the torn areas whenever I have an erection or come into contact with water.
I would like to know if there are any ointments that can help.
As for circumcision, since my foreskin does not appear excessively long when erect, I feel that circumcision may not be necessary.
I truly hope to find a way to resolve this issue.
Thank you.

Managing penile skin tears, particularly in the context you've described, can be a challenging issue that requires careful consideration and appropriate treatment. It sounds like you are experiencing recurrent skin tears in the area where the foreskin has been retracted, leading to discomfort and bleeding during erections. This condition can be quite distressing, and it's important to address it effectively.

Understanding the Condition
The skin of the penis, particularly the foreskin and glans (the head of the penis), is sensitive and can be prone to injury, especially if there is friction or if the skin is not adequately lubricated. When the foreskin is retracted and does not return to its original position, it can create folds or wrinkles that may become taut during an erection. This tension can lead to skin tears, which can be painful and may take time to heal.

Treatment Options
1. Topical Treatments:
- Hydrocortisone Cream: A mild topical steroid can help reduce inflammation and promote healing. However, it should be used sparingly and not for prolonged periods, as it can thin the skin over time.

- Antibiotic Ointments: If there are signs of infection or if the skin tears are open, applying a topical antibiotic ointment like Neosporin can help prevent infection and promote healing.

- Moisturizers: Using a gentle, fragrance-free moisturizer can help keep the skin hydrated and reduce friction. Products containing hyaluronic acid or glycerin can be beneficial.

2. Barrier Methods:
- Lubrication: Using a water-based or silicone-based lubricant during sexual activity can help reduce friction and prevent further tearing. This is especially important if you are experiencing discomfort during erections.

- Condoms: Wearing a condom can provide a protective barrier that may help reduce friction and prevent skin tears.

3. Hygiene Practices:
- Maintaining good hygiene is crucial. Gently cleaning the area with mild soap and water can help prevent irritation and infection. Avoid harsh soaps or products that may irritate the sensitive skin.

4. Consultation with a Specialist:
- Given the recurrent nature of your skin tears, it may be beneficial to consult with a urologist or dermatologist who specializes in genital skin conditions. They can provide a thorough examination and may suggest additional treatments, such as prescription creams or other interventions.

5. Consideration of Circumcision:
- While you mentioned that your foreskin does not appear to be excessively long, circumcision may be a consideration if conservative treatments do not provide relief. This procedure can eliminate the issue of the foreskin retracting and causing friction, thus preventing further skin tears. However, this is a personal decision that should be made in consultation with a healthcare provider.

Conclusion
In summary, managing penile skin tears requires a combination of topical treatments, lubrication, and possibly a consultation with a specialist. It's essential to address the underlying issues contributing to the skin tears to prevent recurrence. If conservative measures do not yield satisfactory results, discussing surgical options with a healthcare provider may be warranted. Remember, open communication with your healthcare provider about your symptoms and concerns is key to finding an effective solution.
