Recurrent injury of the frenulum?
Hello doctor, recently I've been experiencing frequent skin tears or bleeding around the frenulum, usually occurring after masturbation.
Additionally, I feel a stinging sensation when water touches that area during showers.
This situation has happened multiple times.
The frenulum feels thinner, the skin in that area is very thin, and it feels tight during erections.
I have a bit of phimosis, but I can retract the foreskin fully while showering.
If I don't want to undergo circumcision, how can I address this issue? Is it possible to specifically lengthen the frenulum? Thank you.

Hello: Just keep the area clean and dry to avoid causing a wound, as repeated injuries can lead to scarring.
If you are considering procedures for extension or similar treatments, it may be best to consult a physician who specializes in such surgeries.
Wishing you good health.

Managing recurrent injuries of the frenulum, particularly in the context of the foreskin and its associated structures, can be a challenging issue for many individuals. The frenulum, a band of tissue connecting the underside of the penis to the foreskin, plays a crucial role in sexual function and sensation. When it becomes damaged or inflamed, it can lead to discomfort, pain, and bleeding, particularly during activities such as masturbation or sexual intercourse.
From your description, it sounds like you are experiencing recurrent tearing and bleeding of the frenulum, which may be exacerbated by friction during masturbation. This can be a result of several factors, including the tightness of the frenulum, the thickness of the skin in that area, and possibly the presence of phimosis (a condition where the foreskin cannot be fully retracted over the glans).
Here are some insights and potential solutions to manage this condition:
1. Gentle Care and Hygiene: It’s essential to maintain good hygiene in the genital area. After any activity that may cause friction, such as masturbation, gently clean the area with mild soap and water. Avoid harsh soaps or products that may irritate the skin further.
2. Moisturizing: Applying a gentle, fragrance-free moisturizer or a barrier cream to the area may help keep the skin supple and reduce the likelihood of tearing. Products containing aloe vera or vitamin E can be soothing and may promote healing.
3. Avoiding Irritation: During sexual activity or masturbation, consider using lubrication to reduce friction. Water-based lubricants are generally safe and effective. Avoiding excessive force or aggressive techniques can also help prevent further injury.
4. Frenuloplasty: If the frenulum is too tight or if you continue to experience recurrent injuries, a surgical procedure known as frenuloplasty may be an option. This procedure involves lengthening the frenulum to reduce tension and the risk of tearing. It is a relatively minor procedure and can be performed by a urologist.
5. Consultation with a Urologist: Given your symptoms and the recurrent nature of the problem, it would be beneficial to consult with a urologist. They can assess the condition of your frenulum and foreskin and discuss the best treatment options tailored to your situation. They may also evaluate whether a circumcision is necessary, although you mentioned a preference to avoid this.
6. Phimosis Management: If you have phimosis, which may be contributing to the tightness and discomfort, there are non-surgical options available. Topical corticosteroids can sometimes help to loosen the foreskin over time. Stretching exercises may also be recommended, but these should be done carefully and under guidance to avoid injury.
7. Monitoring for Infection: If you notice any signs of infection, such as increased redness, swelling, or discharge, it’s crucial to seek medical attention promptly. Infections can complicate healing and lead to more significant issues.
8. Avoiding Sexual Activity During Healing: If you experience a tear or injury, it’s advisable to refrain from sexual activity until the area has healed completely. This will help prevent further irritation and allow the tissue to recover.
In summary, managing recurrent injuries of the frenulum involves a combination of good hygiene practices, the use of lubricants, and possibly medical interventions if the problem persists. Consulting with a healthcare professional, particularly a urologist, will provide you with tailored advice and treatment options that can help alleviate your symptoms and improve your quality of life.
