Recurrence of hemorrhoids?
Two years ago, I underwent a circumferential excision surgery due to internal and external hemorrhoids.
However, I later discovered that my external hemorrhoids were not completely removed.
Recently, due to poor sleep and constipation, my external hemorrhoids have recurred.
Currently, I have been doing warm sitz baths and applying ointment.
Although it is not very severe at the moment, I really dislike the strange feeling of having something in my anal area.
Are there suitable outpatient surgical options for treatment?
Mr. Wu, 30~39 year old female. Ask Date: 2011/10/13
Dr. Ke Fangxu reply Surgery
After a circular resection surgery, recurrent hemorrhoids are difficult to manage because there is a ring of sutures inside the anal canal, which complicates any further treatment.
It is likely that there are no suitable outpatient procedures that can be performed.
Additionally, what do you mean by recurrence? If it is pain, topical medications can be used for treatment.
However, if it is hemorrhoidal prolapse, then it may be necessary to undergo another traditional surgical procedure.

Managing recurring hemorrhoids can be quite challenging, especially after previous surgical interventions. Hemorrhoids, whether internal or external, can cause discomfort, pain, and even bleeding, significantly affecting one's quality of life. Given your history of a circumferential excision and the recent recurrence of external hemorrhoids, it’s essential to explore effective treatment options that can alleviate your symptoms and prevent further complications.
Firstly, it’s good to hear that you are already practicing self-care measures such as warm sitz baths and topical ointments. These conservative treatments can help soothe the area, reduce inflammation, and promote healing. However, if your symptoms persist or worsen, it may be time to consider more definitive treatment options.
1. Minimally Invasive Procedures: There are several minimally invasive procedures that can be effective for treating recurring hemorrhoids. These include:
- Rubber Band Ligation: This is a common outpatient procedure where a small rubber band is placed around the base of the hemorrhoid to cut off its blood supply. The hemorrhoid will eventually shrink and fall off within a week or so.
- Sclerotherapy: In this procedure, a chemical solution is injected into the hemorrhoid, causing it to shrink. This is often used for smaller hemorrhoids.
- Infrared Coagulation: This technique uses infrared light to coagulate the blood vessels supplying the hemorrhoid, leading to its shrinkage.
2. Surgical Options: If your external hemorrhoids are large or if you have persistent symptoms despite conservative treatment, surgical options may be warranted. These include:
- Hemorrhoidectomy: This is a surgical procedure to remove the hemorrhoids. It is usually recommended for severe cases or when other treatments have failed. Recovery can be painful, but it is often effective in providing long-term relief.
- Stapled Hemorrhoidopexy: This is a less painful alternative to traditional hemorrhoidectomy. It involves using a stapling device to reposition the hemorrhoids and cut off their blood supply.
3. Lifestyle Modifications: In addition to medical treatments, lifestyle changes can play a crucial role in managing hemorrhoids. Consider the following:
- Dietary Changes: Increasing your fiber intake can help prevent constipation, which is a significant contributor to hemorrhoid formation. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Additionally, staying well-hydrated is essential.
- Regular Exercise: Engaging in regular physical activity can help improve bowel function and reduce the risk of constipation.
- Avoid Straining: When using the bathroom, avoid straining or sitting for prolonged periods, as this can exacerbate hemorrhoid symptoms.
4. Consultation with a Specialist: Given your previous surgery and current symptoms, it would be beneficial to consult a colorectal surgeon or a gastroenterologist who specializes in hemorrhoid treatment. They can evaluate your condition and recommend the most appropriate treatment based on the severity of your symptoms and your medical history.
In summary, while conservative measures can provide temporary relief, it’s essential to consider more definitive treatments if your symptoms persist. Minimally invasive procedures or surgical options may be necessary to address the underlying issues effectively. Additionally, lifestyle modifications can significantly contribute to preventing future occurrences. Always consult with a healthcare professional to determine the best course of action tailored to your specific situation.
