Post-hemorrhoid surgery
Hello Doctor: I underwent traditional excision of internal and external hemorrhoids on September 29.
Now, fifteen days post-surgery, my wound has generally healed well.
However, after the surgery, there is a lump at the anal opening that resembles an external hemorrhoid (but it is not very painful).
At that time, the doctor mentioned it was swelling from the surgery.
However, during my follow-up appointment yesterday, the doctor noted that the swelling had not subsided and needed to push the swollen area back into the anal canal.
I asked the doctor if this could be a recurrence of hemorrhoids, but the doctor was reluctant to give me a direct answer, only stating that we should observe it.
I would like to inquire if it is normal for this swelling not to have disappeared after half a month post-surgery, or if it has indeed turned into a recurring hemorrhoid?

Sometimes, during hemorrhoidectomy, a portion of skin may be left to prevent anal stenosis, which could result in excess skin after the surgery.
If there is no pain or bleeding, it can be ignored.
If treatment is desired, an outpatient surgical excision can be performed depending on the situation.

After undergoing a traditional hemorrhoidectomy, it is not uncommon for patients to experience some swelling or a lump around the anal area as part of the healing process. This swelling can be attributed to the surgical trauma and the body's natural inflammatory response. However, the persistence of this swelling beyond the typical recovery period can be concerning and warrants further evaluation.
In your case, it has been fifteen days since your surgery, and while the wound appears to be healing well, the lump you describe at the anal opening is still present. Your surgeon's initial assessment that this is post-operative swelling is reasonable, but the fact that it has not resolved after two weeks raises some questions. It is important to differentiate between normal post-surgical swelling and the potential recurrence of hemorrhoids or the development of other complications.
The lump you are experiencing could be a few different things:
1. Post-Surgical Edema: This is the most common cause of swelling after surgery. It typically resolves on its own as the body heals. However, if the swelling persists beyond a few weeks, it may need to be reassessed.
2. Thrombosed Hemorrhoid: Sometimes, a thrombosed hemorrhoid can occur post-surgery, especially if there is increased pressure in the anal area due to straining or constipation. This can present as a painful lump and may require treatment.
3. Skin Tags: After hemorrhoid surgery, some patients may develop skin tags, which are benign and can appear as small lumps around the anal area. These are usually not painful but can be bothersome.
4. Infection or Abscess: If the lump is accompanied by increased pain, redness, or discharge, it could indicate an infection or abscess, which would require prompt medical attention.
5. Recurrence of Hemorrhoids: Although less likely shortly after surgery, it is possible for hemorrhoids to recur. If the lump resembles a hemorrhoid, it may need to be evaluated further.
Given that your surgeon has suggested monitoring the situation, it is crucial to maintain open communication with them. If you notice any changes, such as increased pain, changes in bowel habits, or any signs of infection, you should contact your healthcare provider immediately.
In the meantime, here are some general recommendations to aid in your recovery:
- Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation, which can exacerbate hemorrhoid symptoms. Consider incorporating fruits, vegetables, and whole grains into your meals.
- Hydration: Drink plenty of fluids to help keep your stools soft.
- Warm Sitz Baths: Taking warm baths can help soothe the area and reduce discomfort.
- Avoid Straining: When having a bowel movement, try not to strain, as this can increase pressure in the anal area.
- Follow-Up Appointments: Keep all scheduled follow-up appointments with your surgeon to monitor your recovery and address any concerns.
In conclusion, while some swelling after hemorrhoid surgery is normal, the persistence of a lump warrants further evaluation. It is essential to discuss your concerns with your surgeon, who can provide a thorough examination and determine the best course of action. Remember, your recovery is a priority, and addressing any issues early can lead to better outcomes.
