Abdominal bloating. Sharp pain in the upper right abdomen. Sensation of something being trapped?

Hello, doctor.
I have been experiencing bloating for the past seven months, along with a sensation of fullness, mild stabbing pain, and burning in the upper right abdomen.
Every time I eat, even when consuming liquids like protein shakes, I feel a foreign body sensation and stabbing pain in the upper right abdomen, followed by bloating.
I also have gas in both the large and small intestines.
A CT scan has shown the presence of ascites, and different hospitals have suggested repeating the CT scan.
I'm unsure if I should proceed with that.
The findings indicate degenerative changes in the bony structures with spur formation, visible shadows of the bilateral psoas muscles, fecal material retention in the large bowel loops, and gaseous dilatation of the bowel loops.
Subtle lesions may be imperceptible in this exam, and other imaging modalities are advised if clinically indicated.
Many doctors have said there is no issue, attributing it to excessive fecal matter.
However, I still experience bloating in the upper right abdomen and am concerned about the possibility of an obstruction or if any organ might be trapped.

Hello, Zhang Cheng:
You have been experiencing bloating for the past seven months, along with a sensation of fullness, mild stabbing pain, and burning in the upper right abdomen.
After eating, even when consuming liquids like protein shakes, you feel a foreign body sensation and stabbing pain in the upper right abdomen, accompanied by bloating.
Additionally, you have gas in both the large and small intestines.
A CT scan has shown the presence of ascites, and different hospitals have suggested another CT scan.
It is unclear whether you should proceed with that.
Your condition does not appear to be complex, and without a complete medical history and imaging reports to support it, I recommend visiting a gastroenterology and hepatology specialist to help alleviate your discomfort.
Thank you!

More Info

Upper right abdominal pain and bloating can be quite distressing, and understanding the potential causes and solutions is essential for effective management. Based on your description, it seems you have been experiencing discomfort for several months, characterized by a sensation of fullness, sharp pain, and bloating, particularly after eating or drinking. Additionally, your imaging studies have indicated some issues, including fecal retention and gaseous dilation in the bowel loops, which could be contributing to your symptoms.

Potential Causes
1. Gastrointestinal Issues:
- Irritable Bowel Syndrome (IBS): This condition can lead to bloating and abdominal pain, often triggered by certain foods or stress. Symptoms can vary widely among individuals.

- Constipation: Retained fecal material can lead to significant discomfort and bloating. The CT scan findings suggest that fecal retention may be a contributing factor.

- Gastritis or Peptic Ulcer Disease: Inflammation of the stomach lining or ulcers can cause pain and a burning sensation, especially after meals.

- Gallbladder Disease: Given your location of pain, gallbladder issues, including gallstones or cholecystitis, could be a possibility, especially if you have a history of gallbladder problems.

2. Biliary Tract Issues:
- Biliary Colic: This occurs when a gallstone temporarily blocks the bile duct, leading to pain that can be sharp and may radiate to the back or shoulder.

- Cholecystitis: Inflammation of the gallbladder can cause significant pain in the upper right abdomen, often accompanied by nausea and vomiting.

3. Musculoskeletal Pain:
- Sometimes, pain in the upper right abdomen can be referred from musculoskeletal issues, such as muscle strain or rib dysfunction, especially if the pain is exacerbated by movement or certain positions.

4. Other Considerations:
- Pancreatitis: Inflammation of the pancreas can cause upper abdominal pain and may be associated with eating.

- Liver Conditions: Given your history of chronic hepatitis B, liver-related issues should be considered, although your liver function tests appear stable.

Recommended Actions
1. Further Evaluation:
- Since you have already undergone imaging studies, it may be beneficial to consult a gastroenterologist who can provide a comprehensive evaluation of your symptoms and possibly recommend additional tests, such as an upper endoscopy or more advanced imaging (like an MRI or MRCP) to assess the biliary tree and pancreas.

2. Dietary Modifications:
- Keeping a food diary may help identify any specific triggers for your symptoms. Consider a low-FODMAP diet, which can be beneficial for individuals with IBS and bloating.

3. Medications:
- Over-the-counter medications such as simethicone can help alleviate gas and bloating. If constipation is an issue, a stool softener or laxative may be necessary.

- If gastritis or ulcers are suspected, proton pump inhibitors (PPIs) or H2 blockers may be prescribed by your doctor.

4. Lifestyle Changes:
- Regular exercise can help improve gastrointestinal motility and reduce bloating. Additionally, managing stress through relaxation techniques may also be beneficial.

5. Follow-Up:
- If your symptoms persist or worsen, it is crucial to seek further medical attention. Continuous monitoring and follow-up with your healthcare provider are essential to ensure that any underlying conditions are addressed promptly.

In conclusion, while your imaging studies suggest that fecal retention may be a significant factor, the complexity of abdominal pain necessitates a thorough evaluation to rule out other potential causes. Collaborating closely with your healthcare team will be vital in managing your symptoms effectively and improving your quality of life.
