Dyspnea, valvular insufficiency, sleep disturbances?
Hello, doctor.
Four years ago, I was diagnosed with arrhythmia.
Recently, my doctor diagnosed me with mild mitral valve regurgitation and cardiac spasms.
I have undergone a 24-hour Holter monitor, dynamic EKG, and echocardiogram, and was told that there is nothing serious.
I am currently taking Bisoprolol (one in the morning and one at night).
However, this past week, I have been experiencing worsening shortness of breath (deep inhalation and wheezing sounds), and I cannot breathe normally while lying down.
At night, I have to sleep in a semi-upright position until I fall asleep.
Could this be caused by the mitral valve regurgitation? Should I return to the hospital for further examination? It has been quite frustrating as they have not been able to determine the cause each time.
I appreciate your guidance!

Hello, have you experienced wheezing while breathing before? Are you having difficulty breathing and unable to breathe normally while lying down? It is advisable to return for a check-up.

Shortness of breath, especially when it becomes more pronounced and persistent, can be a concerning symptom that warrants further evaluation. In your case, you have a history of arrhythmia and mild valve insufficiency, which could be contributing factors to your current respiratory difficulties.
Valve insufficiency, particularly mitral or aortic regurgitation, can lead to increased pressure in the heart and lungs, potentially causing pulmonary congestion. This congestion can manifest as shortness of breath, especially when lying down, as gravity can exacerbate fluid accumulation in the lungs. The fact that you are experiencing wheezing sounds and difficulty breathing while lying flat suggests that there may be some degree of fluid buildup or increased pressure in your pulmonary circulation.
Your recent symptoms of worsening shortness of breath, particularly the need to sleep in a semi-upright position, are significant. This positional dyspnea (difficulty breathing when lying flat) can indicate worsening heart function or increased pulmonary pressure, which may be related to your valve insufficiency or other cardiac issues. The wheezing sounds you describe could also suggest bronchospasm or airway constriction, which can occur in conjunction with heart problems or may be a separate respiratory issue.
Given your history of heart issues and the recent escalation of your symptoms, it is advisable to seek medical attention promptly. A follow-up with your cardiologist or a visit to the emergency department may be warranted, especially if you experience any additional symptoms such as chest pain, palpitations, or significant fatigue.
In terms of diagnostic evaluations, your healthcare provider may consider repeating echocardiography to assess the current status of your heart valves and overall cardiac function. They may also recommend additional tests, such as a chest X-ray or CT scan, to evaluate for any pulmonary issues or fluid accumulation in the lungs.
Furthermore, it is essential to discuss your current medication regimen with your physician. While you are taking a beta-blocker (likely to manage your heart rate and arrhythmia), it is crucial to ensure that your medications are appropriately adjusted to manage both your heart condition and any respiratory symptoms you may be experiencing.
In summary, the worsening of your shortness of breath, especially with the need to sleep in an upright position, is concerning and should not be ignored. It is essential to seek medical evaluation to determine the underlying cause and to ensure that appropriate management strategies are in place to address both your cardiac and respiratory health.
