Frequent urination issues?

Hello Doctor, this past Monday I visited a gynecologist due to issues with a sebaceous cyst in the genital area, frequent urination, and pain upon contact with the urethra.
The doctor mentioned that my discomfort was caused by irritation of the urethra from secretions (urination itself does not hurt) and prescribed Meikang ointment to apply to the genital area.
While the frequent urination has improved, I now feel that the area has become more sensitive after using the ointment; even slight contact with the urethral opening causes pain (originally, it only hurt occasionally).
Could you please explain why this is happening? Should I stop using the ointment? Thank you.

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, please stop using the ointment and it is recommended to visit the obstetrics and gynecology department for further evaluation.
Wishing you good health.

Frequent urination and sensitivity in the genital area can be distressing and may stem from various underlying issues. In your case, it seems that the initial diagnosis pointed towards irritation of the urethra due to secretions, which is a common cause of discomfort. The treatment with a topical ointment, such as the one you mentioned (美康藥膏), may have provided some relief for the frequent urination, but it appears to have led to increased sensitivity and pain upon contact with the urethral opening.

The increased sensitivity you are experiencing after applying the ointment could be attributed to several factors. First, the active ingredients in the ointment may cause localized irritation, especially if the skin in that area is already sensitive or inflamed. This is not uncommon, as topical medications can sometimes lead to a paradoxical increase in sensitivity, particularly if they contain ingredients that may not be well-tolerated by everyone.

Additionally, the skin and mucous membranes in the genital area are delicate and can react to various substances, including medications, soaps, or even friction from clothing. If the ointment is causing discomfort, it may be wise to discontinue its use and consult your healthcare provider for further evaluation. They may recommend an alternative treatment or suggest a different approach to manage the sensitivity and discomfort.

It's also important to consider that frequent urination can be a symptom of several conditions, including urinary tract infections (UTIs), bladder overactivity, or even irritation from external factors like soaps or hygiene products. Since your symptoms improved with the ointment but then worsened with increased sensitivity, it may be beneficial to explore other potential causes of your symptoms.
If you have not already done so, I recommend keeping a diary of your symptoms, noting when they occur, their severity, and any potential triggers. This information can be invaluable for your healthcare provider in determining the best course of action.
In terms of further evaluation, if your symptoms persist or worsen, consider asking for a referral to a urologist or a specialist in pelvic floor disorders. They can perform more comprehensive assessments, such as urodynamic studies, to evaluate how your bladder and urethra are functioning.
In summary, while the ointment may have initially helped with frequent urination, the subsequent increase in sensitivity suggests that it may not be the right treatment for you. Discontinuing the ointment and consulting with your healthcare provider for further evaluation and alternative treatments is advisable. Remember, your comfort and well-being are paramount, and addressing these symptoms with a healthcare professional can lead to a more effective management plan tailored to your needs.
