Double vision issue?
Hello, doctor.
1.
Today, I suddenly noticed that I see double when looking at things.
The text on my phone appears to be significantly overlapping, but text in reality does not.
When I look at certain objects, I also see some overlapping, although the extent is not large.
The strangest part is that I just had an eye examination today, where the doctor used dilation drops and confirmed that my eyes are very healthy with no issues.
So what could be the problem?
2.
I've also noticed recently that when I look at lights, there are faint rays emanating from them.
Today, I went to check if my astigmatism has worsened, but I was told that there hasn't been much change in my astigmatism either.
I’m not sure why these issues are occurring.

Dr. Gao Pilin reply Ophthalmology
Hello Mr.
Chen: If the double vision occurs when viewing with one eye, it is likely a refractive issue.
If it occurs when viewing with both eyes, it may be due to diplopia.
The underlying cause will need to be determined after an examination.

More Info
Double vision, or diplopia, can be a perplexing and concerning symptom for many individuals. Understanding its causes and implications is crucial for effective management and peace of mind. Based on your description, it seems you are experiencing a form of double vision that is particularly noticeable when looking at text on your phone, while not affecting your perception of real-world objects. This phenomenon can be attributed to several factors, and I will outline them below.
Possible Causes of Double Vision
1. Refractive Errors: One common cause of double vision, especially in individuals with existing vision issues, is uncorrected refractive errors such as myopia (nearsightedness), hyperopia (farsightedness), or astigmatism. Even if your recent eye exam showed no significant changes in your prescription, slight discrepancies in how your eyes focus can lead to temporary diplopia, particularly when viewing objects at close range.
2. Eye Muscle Imbalance: Another potential cause is an imbalance in the muscles that control eye movement. This can lead to misalignment of the eyes, causing one eye to deviate slightly from the other. This misalignment can result in double vision, especially when focusing on nearby objects. Although your eye exam did not reveal any significant issues, it may be worthwhile to have a comprehensive evaluation of your eye muscle function.
3. Fatigue or Eye Strain: Prolonged screen time, particularly with the increased use of digital devices, can lead to eye strain and fatigue. This can manifest as temporary double vision, especially if your eyes are not able to converge properly when focusing on close objects. Taking regular breaks and practicing the 20-20-20 rule (every 20 minutes, look at something 20 feet away for 20 seconds) can help alleviate this issue.
4. Neurological Factors: In some cases, double vision can be a sign of neurological issues, such as problems with the cranial nerves that control eye movement. While your recent eye exam was normal, if the double vision persists or worsens, it may be prudent to consult a neurologist for further evaluation, including imaging studies if necessary.
5. Corneal or Lens Issues: Conditions affecting the cornea or lens, such as cataracts or corneal irregularities, can also lead to visual distortions. Although your eye health was deemed normal, it’s essential to keep an eye on any changes in your vision, especially if you notice new symptoms.
Recommendations for Management
- Follow-Up with an Eye Specialist: Since you have already seen an eye doctor, consider scheduling a follow-up appointment if the double vision persists. A specialist may conduct additional tests, such as a cover test or prism testing, to evaluate eye alignment and function more thoroughly.
- Monitor Symptoms: Keep a diary of when the double vision occurs, its duration, and any associated symptoms (like headaches or fatigue). This information can be valuable for your healthcare provider in diagnosing the issue.
- Consider Vision Therapy: If an eye muscle imbalance is suspected, vision therapy may be beneficial. This therapy involves exercises designed to improve coordination and focusing abilities of the eyes.
- Lifestyle Adjustments: Reducing screen time, ensuring proper lighting while reading, and taking regular breaks can help reduce eye strain and improve overall comfort.
Conclusion
In summary, while your recent eye examination indicated no significant issues, the experience of double vision can arise from various factors, including refractive errors, muscle imbalances, or fatigue. It is essential to monitor your symptoms and seek further evaluation if they persist. Understanding the underlying causes can help alleviate concerns and guide you toward effective management strategies. If you have any further questions or if your symptoms change, do not hesitate to reach out to your eye care provider for additional guidance.
